光激化学发光与电化学发光分析法测定儿童TSH相关性评价 被引量:1

Correlation Between Photoluminescence and Electrochemiluminescence in the Determination of TSH in Children

摘要 目的了解光激化学发光与电化学发光分析法在测定促甲状腺激素(TSH)不同水平区间的相关性,评价并探讨两种检测方法的临床检测实用价值。方法选取昆明医科大学附属儿童医院2017年1月至12月内分泌科检测甲状腺功能的122例患儿作为研究对象,根据TSH检测结果按照不同浓度区间划分为低值组(0.004~0.410 mU/L)、中值组(0.410~7.000 mU/L)、高值组(7.000~60.000 mU/L)。将光激化学发光与电化学发光分析法测定值比进行相关分析和一致性(Kappa)检验,计算两组数据的符合率和Kappa值。结果两种检测方法在低值组、中值组及高值组三组的Roche E602数据,以及ChemclinLiCA500的比较差异均无统计学意义(P>0.05)。两种方法TSH检测结果显示,低值组数据符合率为83.33%,中值组数据符合率为92.59%,高值组数据符合率为82.14%,总符合率为86.89%(Kappa=0.783)。两种方法TSH检测结果中,不同评价区间有良好的一致性。结论国产光激化学发光分析技术在TSH检测的不同区间值与电化学发光分析技术有相近的检测性能,能够运用于儿童TSH的检测中。 Objective To investigate the correlation between photoluminescence and electrochemiluminescence in different levels of TSH,and to evaluate the clinical value of the two methods.Methods A total of 122 children who underwent thyroid function testing in Children's Hospital Affiliated to Kunming Medical University from January to December 2017 were selected as the study subjects,and were divided into low value group(0.004~0.410 mU/L),median group(0.410~7.000 mU/L)and high value group(7.000~60.000 mU/L)according to the results of TSH detection according to different concentration intervals.Correlation analysis and Kappa test were carried out to calculate the coincidence rate and Kappa value of the two groups of data.Results There was no significant difference in Roche E602 data and ChemclinLiCA500 between the two detection methods in the low,median and high value groups(P>0.05).In addition,the two methods of TSH test results,the low group data coincidence rate was 83.33%,the median group data coincidence rate was 92.59%,the high group data coincidence rate was 82.14%,the total coincidence rate was 86.89%(Kappa=0.783).The TSH test results of the two methods have good consistency among different evaluation intervals.Conclusion The domestic photoluminescence analysis technology has similar detection performance with the electrochemiluminescence analysis technology in different intervals of TSH detection,which can be used in children's TSH detection.
