摘要
目的探讨瑞芬太尼复合异丙酚对肝脏肿瘤微波消融术围术期体征的影响。方法选取2018年11月至2019年4月广州现代医院收治的肝脏肿瘤微波消融术患者86例作为研究对象,按照随机数字表法分为A组与B组,各43例。A组采用瑞芬太尼复合异丙酚,B组采用舒芬太尼复合异丙酚。对两组患者的术前(T0)、手术开始时(T1)、手术开始5 min(T2)、手术20 min(T3)、手术结束时(T4)、术后苏醒(T5)的平均动脉压(MAP)、心率(HR)指标进行记录,并比较两组患者麻醉不良反应的发生率。结果两组患者T0时间段的HR及MAP水平比较,差异无统计学意义(P>0.05);A组患者T1、T2、T3、T4、T5时间段的HR及MAP与B组比较,差异有统计学意义(P<0.05);A组患者苏醒期躁动、恶心呕吐、低血压的发生率为6.98%,显著低于B组的23.26%(P<0.05)。结论瑞芬太尼复合异丙酚用于肝脏肿瘤微波消融术中能减少患者体征指标的波动,安全性更高。
Objective To investigate the effect of remifentanil combined with propofol on perioperative signs of hepatic tumor microwave ablation.Methods Totally 80 patients who underwent microwave ablation of liver tumors in Guangzhou Modern Hospital from November 2018 to April 2019 were selected as the study subjects and divided into group A and group B according to the random number table method,with 43 patients in each group.Remifentanil combined with propofol was used in group A and sufentanil combined with propofol in group B.The mean arterial pressure(MAP)and heart rate(HR)were recorded before operation(T0),at the beginning of operation(T1),5_min at the beginning of operation(T2),20 min at the beginning of operation(T3),at the end of operation(T4),and after awakening(T5)in the two groups,and the incidence of adverse reactions to anesthesia was compared between the two groups.Results There was no significant difference in HR and MAP levels between the two groups at time T0(P>0.05).While there was A statistically significant difference in HR and MAP levels at time T1,T2,T3,T4 and T5 between group A and group B(P<0.05).The incidence of restlessness,nausea and vomiting and hypotension in group A was 6.98%,which was significantly lower than that in group B 23.26%(P<0.05).Conclusion Remifentanil combined with propofol in microwave ablation of liver tumors can reduce the fLuctuation of signs and indicators,and has higher safety.
