腹腔镜与开腹阑尾切除术在急性穿孔性阑尾炎治疗中的疗效分析

Analysis of the curative effect of laparoscopic and open appendectomy in the treatment of acute perforating appendicitis

目的探究急性穿孔性阑尾炎腹腔镜与开腹阑尾切除术治疗的疗效。方法选取2018年10月至2019年10月本院病历保存完整的118例急性穿孔性阑尾炎患者作为研究对象,依据随机分组法分为研究组与参照组,各59例。参照组接受开腹阑尾切除术,研究组接受腹腔镜阑尾切除术,评价两组手术情况。结果研究组治疗总有效率为98.31%,高于参照组的88.14%(P<0.05);研究组出血量均少于参照组,手术时间、切口长度及住院时间均短于参照组,术后排气时间及进食时间早于参照组(P<0.05);研究组术后IL-6、PCT以及CRP水平均低于参照组(P<0.05);研究组并发症发生率为3.39%,低于参照组的15.25%(P<0.05)。结论采取腹腔镜阑尾切除术治疗急性穿孔性阑尾炎效果显著,能够保证患者术后各项指标稳定,同时还可降低相关并发症的发生率,是一种安全、可靠的手术治疗方式。

Objective To explore the effect of laparoscopic and open appendectomy for acute perforating appendicitis.Methods A total of 118 patients with acute perforating appendicitis with complete medical records kept in our hospital from October 2018 to October 2019 were selected as the study subjects.According to the randomization method,they were divided into study group and reference group,with 59 cases in each group.The reference group underwent open appendectomy,and the study group underwent laparoscopic appendectomy.The operation conditions of the two groups were evaluated.Results The total effective rate of treatment in the study group was 98.31%,which was higher than 88.14%in the reference group(P<0.05).The blood loss in the study group was less than that in the reference group,and the operation time,incision length and hospital stay were shorter than those in the reference group.Exhaust time and eating time were earlier than the reference group(P<0.05).The postoperative IL-6,PCT and CRP levels in the study group were lower than those in the reference group(P<0.05).The complication rate in the study group was 3.39%,which was lower than that 15.25%in the reference group(P<0.05).Conclusion Laparoscopic appendectomy has obvious advantages in the treatment of acute perforating appendicitis.It can ensure the stability of the postoperative indicators of the patient and reduce the incidence of related complications.It is a safe and reliable surgical treatment.
