社区慢性病管理模式在老年高血压管理中的效果评价

Evaluation of the effect of community chronic disease management model in the management of elderly people with hypertension

目的:探讨社区慢性病管理模式在老年高血压管理中的效果。方法:选取2017年3月-2019年3月社区老年高血压患者200例,随机分为两组,各100例。对照组给予常规基础干预;观察组给予社区慢性管理模式。比较两组患者管理后血压控制情况和满意度。结果:观察组管理12个月后收缩压和舒张压水平均低于对照组,差异有统计学意义(P<0.05)。观察组患者满意度高于对照组,差异有统计学意义(P<0.05)。结论:社区慢性病管理模式应用在老年高血压管理中,有利于改善患者血压水平,提高护理效果。

Objective:To explore the effect of community chronic disease management model in the management of elderly people with hypertension.Methods:From March 2017 to March 2019,200 cases of elderly hypertension patients were enrolled,they were randomly divided into two groups with 100 cases in each group.The control group was given conventional basic intervention.The observation group was given community chronic management model.The blood pressure control and satisfaction of patients after management were compared between the two groups.Results:The levels of systolic and diastolic blood pressure of the observation group were lower than those of control group after 12 months of management,the differences were statistically significant(P<0.05).The patients'satisfaction in observation group was higher than that in control group,the difference was statistically significant(P<0.05).Conclusion:The application of community chronic disease management model in the management of elderly hypertension patients is beneficial to improve patients'blood pressure level and it can improve nursing effect.
