持续颅内压监测在大面积脑梗死手术治疗中的临床应用 被引量:4

摘要 目的探讨持续有创颅内压监测在大面积脑梗死患者手术治疗中的临床应用价值。方法 41例急性大面积脑梗死手术患者根据术中是否行有创颅内压监测分为ICP监测组和对照组,比较两组甘露醇使用的次数、总剂量和天数以及随访半年后的NIHSS量表、GOS分级、Barthel指数。结果 ICP监测组患者的甘露醇使用次数、总剂量和天数均明显低于对照组(P<0.05),ICP监测组患者随访半年后NIHSS量表、GOS分级、Barthel指数明显高于对照组(P<0.05)。结论大面积脑梗死患者术后给予持续有创颅内压监测可以准确指导降颅压治疗,及时反映病情变化,有效改善患者预后。 Objective To explore the value of continuous invasive intracranial pressure monitoring in the surgical treatment of large area cerebral infarction. Methods 41 patients with acute large area cerebral infarction were divided into the ICP group and the control group according to whether invasive intracranial pressure monitoring was performed during operation. The using times, dosage and days of mannitol and NIHSS scale, GOS grade and Barthel index after half a year follow-up were compared between the two groups. Results The using times, dosage and days of mannitol in the ICP monitoring group were significantly lower than that of the control group(P < 0.05), and the NIHSS scale, GOS score and Barthel Index after half a year in the ICP monitoring group were significantly higher than that of the control group(P < 0.05). Conclusions Continuous invasive intracranial pressure monitoring in patients with large area cerebral infarction after operation can accurately guide the treatment of decreasing intracranial pressure, which reflect the changes of the disease in a timely manner, and effectively improve the prognosis of patients.
