摘要
目的探讨多层螺旋CT不同扫描剂量在冠状动脉成像中的应用价值。方法选择2017年2月到2019年1月来深圳市宝安中医院影像科接受螺旋CT冠状动脉检查80例患者为研究对象,将其分为两组,各40例。对照组使用常规剂量组为120kV/200m As,观察组使用低剂量组参数为120kV/100m As,比较两组图像质量评分、图像信噪比、有效剂量(ED),比较两组准确性、敏感性、特异性比较及信噪比(SNR)及对比噪声比(CNR)。结果观察组图像信噪比与图像质量评分高于对照组,而有效剂量低于对照组(P<0.05);观察组准确性、敏感性、特异性均高于对照组,差异有统计学意义(P<0.05);观察组信噪比(SNR)及对比噪声比(CNR)均低于对照组(P<0.05)。结论多层螺旋CT适当降低管电压,根据患者BMI情况设定管电流能够降低患者所受的辐射剂量,能够提高CT检查的准确性、敏感性以及特异性,改变多层螺旋CT的扫描剂量在冠状动脉造影成像上具有一定的价值。
Objective To investigate the application value of different scanning doses of multi-slice spiral CT in coronary artery imaging. Methods From February 2017 to January 2019, 80 patients who underwent spiral CT coronary artery examination at the Department of Imaging of Shenzhen Baoan Hospital of Traditional Chinese Medicine were selected as the research subjects, and they were divided into two groups of 40 cases each. The conventional dose group of 120 kV/200 m As was used on the control group, and a low dose group with a parameter of 120 kV/200 m As was used on the observation group. The image quality score, image signal-to-noise ratio, and effective dose(ED)of the two groups were compared. Sensitivity, sensitivity, specificity comparison and signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)were compared. Results The image SNR and image quality score of the observation group were higher than that of the control group, but the effective dose was lower than that of the control group(P<0.05);the accuracy, sensitivity and specificity of the observation group were higher than that of the control group, the difference was statistically significant(P<0.05);the SNR and CNR of the observation group were lower than that of the control group(P<0.05). Conclusions Appropriate reduction of tube voltage and setting of tube current according to BMI can reduce the radiation dose of patients, improve the accuracy, sensitivity and specificity of CT examination, and it can change the scanning dose of multi-slice spiral CT which has certain value in coronary angiography imaging.
