慢性鼻窦炎伴鼻息肉鼻内镜术后苍辛通窍方鼻腔冲洗的效果分析

Analysis of the Effect of Cangxin Tongqiao Decoction Nasal Irrigation for Chronic Rhinosinusitis with Nasal Polyps after Nasal Endoscopic Surgery

目的:分析慢性鼻窦炎伴鼻息肉(chronic rhinosinusitis with nasal polyps,CRSwNP)鼻内镜术后苍辛通窍方鼻腔冲洗的效果。方法:选择2017年1月-2019年8月在笔者所在医院接受鼻内镜手术的42例CRSwNP患者,采用随机数字表法将其分为对照组(21例)和研究组(21例)。所有患者均行功能性鼻内镜手术,对照组术后采用2%高渗盐水鼻腔冲洗,研究组采用4%苍辛通窍方鼻腔冲洗。比较两组黏膜纤毛清除功能、术腔清洁时间和临床症状。结果:对照组术后鼻黏膜纤毛输送速率为(4.88±0.77)mm/min,低于研究组的(5.67±0.66)mm/min,差异有统计学意义(t=3.568,P<0.05)。对照组术腔清洁时间为(2.67±0.86)周,长于研究组的(2.14±0.73)周,差异有统计学意义(t=2.137,P<0.05)。对照组术后VAS症状总分为(9.52±3.52)分,高于研究组的(6.48±3.27)分,差异有统计学意义(t=2.911,P<0.05)。结论:CRSwNP鼻内镜术后采用苍辛通窍方冲洗的效果满意,值得今后临床工作中推广应用。

Objective:To analyze the effect of Cangxin Tongqiao Decoction nasal irrigation for chronic rhinosinusitis with nasal polyps(CRSwNP)after nasal endoscopic surgery.Method:A total of 42 patients with CRSwNP who underwent nasal endoscopic surgery in our hospital from January 2017 to August 2019 were selected and divided into the control group(21 cases)and the study group(21 cases)by random number table method.All patients underwent functional nasal endoscopic surgery.The control group adopt 2%hypertonic saline nasal irrigation,the study group adopt 4%Cangxin Tongqiao Decoction nasal irrigation.The mucociliary clearance function,cavity cleaning time and clinical symptoms were compared between the two groups.Result:The nasal mucociliary transmission rate of the control group was(4.88±0.77)mm/min,which was lower than(5.67±0.66)mm/min of the study group,the difference was statistically significant(t=3.568,P<0.05).The cavity cleaning time in the control group was(2.67±0.86)weeks,which was longer than(2.14±0.73)weeks in the study group,the difference was statistically significant(t=2.137,P<0.05).The total VAS symptom score of the control group was(9.52±3.52)points,which was higher than(6.48±3.27)points of the study group,and the difference was statistically significant(t=2.911,P<0.05).Conclusion:The effect of Cangxin Tongqiao Decoction nasal irrigation for CRSwNP after nasal endoscopic surgery is satisfactory,and it is worthy of clinical promotion in the future.
