超前镇痛联合多模式镇痛用于老年髋部骨折术后镇痛效果及对IL-6、CRP水平的影响

Effect of Advanced Analgesia Combined with Multi-mode Analgesia on Postoperative Analgesia in Elderly Hip Fractures and Its Effect on IL-6 and CRP Levels

目的:探讨老年髋部骨折手术患者实施超前镇痛联合多模式镇痛的效果及对IL-6、CRP水平的影响。方法:择取2019年1月-2020年1月笔者所在医院80例老年髋部骨折手术患者为研究对象,按照随机数字表法将其分为对照组和观察组,各40例。对照组实施常规硬膜外自控镇痛,观察组实施超前镇痛联合多模式镇痛。比较两组不同时间VAS评分及IL-6、CRP水平。结果:观察组术后1、24、72 h、1周VAS评分均低于对照组,差异有统计学意义(P<0.05)。观察组术前1 h、术后1、24、48 h IL-6、CRP水平均低于对照组,差异有统计学意义(P<0.05)。结论:老年髋部骨折手术患者实施超前镇痛联合多模式镇痛,可大幅减轻患者术后疼痛,减轻炎性反应,从而加快术后康复,值得临床参考借鉴。

Objective:To investigate the effect of advanced analgesia combined with multi-mode analgesia in elderly patients undergoing hip fracture surgery and its effect on IL-6 and CRP levels.Method:A total of 80 elderly patients undergoing hip fracture surgery in our hospital from January 2019 to January 2020 were selected as the study subjects,and were divided into the control group and the observation group according to the random number table method,with 40 patients in each group.The control group was treated with conventional epidural controlled analgesia,while the observation group was treated with advanced analgesia combined with multi-mode analgesia.The VAS scores,IL-6 and CRP levels at different times were compared between the two groups.Result:The VAS scores at 1,24,72 h and 1 week after surgery in the observation group were all lower than those in the control group,the differences were statistically significant(P<0.05).The levels of IL-6 and CRP at 1 h before surgery,1,24 and 48 h after surgery in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The application of advanced analgesia combined with multi-mode analgesia in elderly patients undergoing hip fracture surgery can greatly reduce postoperative pain,reduce the inflammatory response,and thus accelerate postoperative rehabilitation,which is worthy of clinical reference.
