经尿道前列腺钬激光剜除术治疗伴有前列腺穿刺病史的BPH患者的效果分析

Effect of Transurethral Holmium Laser Enucleation of the Prostate in BPH Patients with A History of Prostate Puncture

目的:研究经尿道前列腺钬激光剜除术(HoLEP)治疗伴有前列腺穿刺病史的良性前列腺增生(BPH)患者的效果。方法:选取2017年11月-2019年11月于笔者所在医院行HoLEP的前列腺增生患者109例,按照是否存在术前行前列腺穿刺病史分为对照组和观察组,对照组为术前无前列腺穿刺病史的54例,观察组为术前存在前列腺穿刺病史的55例。两组均行HoLEP术。观察两组手术指标,术前、术后3个月疗效指标,以及术后并发症发生情况。结果:两组膀胱冲洗时间、手术时间、尿管留置时间及术中出血量比较差异无统计学意义(P>0.05)。两组术前及术后3个月RUV、PFR、IPSS、QOL比较差异均无统计学意义(P>0.05)。观察组术后并发症发生率(9.09%)与对照组(7.40%)比较差异无统计学意义(P>0.05)。结论:经尿道前列腺钬激光剜除术治疗伴有前列腺穿刺病史的BPH患者可行,并且不会增加手术时间、术中出血量、尿管留置时间及膀胱冲洗时间。

Objective:To study the effect of transurethral holmium laser enucleation of the prostate(HoLEP)in treatment of BPH patients with a history of prostate puncture.Method:A total of 109 patients with prostatic hyperplasia who underwent HoLEP in our hospital from November 2017 to November 2019 were selected and were divided into the control group and the observation group according to whether there was a history of preoperative prostate puncture or not.The control group included 54 patients who had no history of prostate puncture before surgery,while the observation group included 55 patients who had history of prostate puncture before surgery.HoLEP was performed in both groups.Surgical indicators,evaluation of efficacy indicators before and 3 months after operation,and occurrence of postoperative complications were observed in both groups.Result:There were no significant differences in bladder irrigation time,operation time,indwelling time of urinary catheter and intraoperative blood loss between the two groups(P>0.05).There was no significant difference in RUV,PFR,IPSS and QOL between the two groups before and 3 months after operation(P>0.05).There was no significant difference in the incidence of postoperative complications between the observation group(9.09%)and the control group(7.40%)(P>0.05).Conclusion:Transurethral holmium laser enucleation of the prostate is feasible in the treatment of BPH patients with a history of prostate puncture and does not increase the operation time,intraoperative blood loss,indwelling time of urinary catheter and bladder irrigation time.
