升阳除湿防风汤联合西药治疗脾虚湿胜型2型糖尿病性腹泻临床观察

Clinical observation of Shengyang dehumidification Fangfeng decoction combined with Western medicine in the treatment of type 2 diabetic diarrhea with spleen deficiency and dampness

[目的]观察升阳除湿防风汤联合西药治疗2型糖尿病性腹泻的临床疗效。[方法]将70例2型糖尿病性腹泻患者随机分为对照组和观察组,每组35例,观察组脱落1例,对照组脱落2例。对照组接受西医常规治疗,观察组在对照组的基础上接受升阳除湿防风汤治疗。各组均治疗4周,观察各组治疗前后自主排便次数、Bristol大便性状分型评分、中医症状评分、生活质量评分及血空腹血糖、餐后2 h血糖、糖化血红蛋白水平,并评定临床疗效。[结果]治疗后,各组患者自主排便次数、Bristol大便性状分型评分、中医症状评分、生活质量评分及血空腹血糖、餐后2 h血糖、糖化血红蛋白水平均明显降低(P<0.05),且观察组自主排便次数、Bristol大便性状分型评分、中医症状评分、生活质量评分均低于对照组(P<0.05);观察组总有效率为91.18%(31/34),高于对照组的69.70%(23/33,P<0.05)。[结论]升阳除湿防风汤联合西药能明显改善脾虚湿胜型2型糖尿病性腹泻患者的腹泻、中医症状及生活质量,同时可以有效地调节糖代谢水平,临床疗效优于单纯西药治疗。

[Objective] To observe the clinical effect of Shengyang dehumidification Fangfeng decoction combined with Western medicine on type 2 diabetic diarrhea.[Methods]Seventy patients with type 2 diabetic diarrhea were randomly divided into the control group and the observation group,35 cases in each group,1 cases in the observation group and 2 case in the control group.The control group was treated with conventional Western medicine,and the observation group was treated with Shengyang dehumidification and Fangfeng decoction on the basis of the control group.Each group was treated for 4 weeks.The number of spontaneous defecation,Bristol stool character score,TCM symptom score,quality of life score,fasting blood glucose(FBG),2-hour postprandial blood glucose(2-hour PBG),HbA1 c were observed before and after treatment,and the clinical therapeutic effect was evaluated.[Results]After treatment,the number of spontaneous defecation,Bristol defecation character score,TCM symptom score,quality of life score,fasting blood glucose(FBG),2-hour postprandial blood glucose(2-hour PBG),glycosylated hemoglobin(HbA1 c)levels in each group were significantly reduced(P<0.05),and the number of spontaneous defecation,Bristol defecation character score,TCM symptom score,quality of life in the observation group were significantly reduced(P<0.05)The total effective rate of the observation group was 91.18%(31/34),higher than 69.70%(23/33,P<0.05).[Conclusion]Shengyang dehumidification Fangfeng decoction combined with western medicine can significantly improve the diarrhea,TCM symptoms and quality of life of type 2 diabetic diarrhea patients with spleen deficiency and dampness,at the same time,it can effectively regulate the level of glucose metabolism,and its clinical effect is better than that of Western medicine alone.
