角巩膜缘切口对白内障超声乳化人工晶体植入术患者视力、散光及眼局部创伤的影响

Influence of Corneo-scleral Limbus Incision for Vision,Astigmatism and Ocular Local Trauma of Patients with Phacoemulsification and Intraocular Lens Implantation for Cataract

目的:研究角巩膜缘切口对白内障超声乳化人工晶体植入术患者视力、散光及眼局部创伤的影响。方法:选取2017年10月-2019年9月本院收治的92例行超声乳化人工晶体植入术的白内障患者为研究对象,将其根据切口的不同分为对照组和观察组,各46例。对照组采用透明角膜切口,观察组采用角巩膜缘切口。比较两组术后不同时间的裸眼视力、散光程度、手术前后的角膜内皮细胞指标、环形角膜荧光染色评分及泪液炎性因子。结果:观察组术后不同时间的裸眼视力及散光程度均显著优于对照组,差异均有统计学意义(P<0.05);观察组术后不同时间的角膜内皮细胞密度显著高于对照组,六角形细胞比例及变异系数、环形角膜荧光染色评分及泪液炎性因子均显著低于对照组,差异均有统计学意义(P<0.05)。结论:在白内障超声乳化人工晶体植入术中采用角巩膜缘切口对眼局部创伤的控制作用积极,应用价值较高。

Objective:To study the influence of corneo-scleral limbus incision for vision,astigmatism and ocular local trauma of patients with phacoemulsification and intraocular lens implantation for cataract.Method:A total of 92 patients with phacoemulsification and intraocular lens implantation for cataract from October 2017 to September 2019 were chosen as the research objects,they were divided into control group and observation group according to different incisions,46 cases each in each group.Clear corneal incision was used in the control group,and keratosera edge incision was used in the observation group.Naked eye vision,astigmatism,corneal endothelial cell index,circular corneal fluorescence staining score and tear inflammatory factors were compared between the two groups at different time after operation.Result:The visual acuity and astigmatism degree of the naked eye of the observation group at different time after operation were significantly better than those of the control group,the differences were statistically significant(P<0.05).The corneal endothelial cell density in the observation group at different postoperative times were significantly higher than those in the control group,the percentage of hexagonal cells and coefficient of variation,ring corneal fluorescence staining score and tear inflammatory factors were significantly lower than those in the control group,with statistically significant differences(P<0.05).Conclusion:In the cataract phacoemulsification IOL implantation,the keratosera edge incision has a positive effect on the control of local ocular trauma,so it has a high application value.
