自拟加味清热化痰汤治疗痰热蕴肺型AECOPD的疗效观察及对血清炎性标志物的影响

Observation of Effect of Self-made Modified Qingre Huatan Decoction on AECOPD with Phlegm Heat Accumulation and Lung Type and Its Influence on Serum Inflammatory Markers

目的:探讨自拟加味清热化痰汤治疗痰热蕴肺型AECOPD的效果及对血清炎性标志物的影响。方法:选取2018年12月-2020年1月在本院接受治疗的痰热蕴肺型AECOPD患者80例为研究对象,按照随机数字表法分为观察组和对照组,各40例。对照组采用西医常规标准治疗,观察组则在对照组基础上联合加味清热化痰汤治疗。比较两组患者治疗前后的血清炎性指标、血气分析指标,观察两组的临床疗效。结果:治疗后,两组的PCT、CRP水平均较治疗前下降,且观察组的PCT、CRP水平均明显低于对照组,差异均有统计学意义(P<0.05)。治疗后,观察组的PaCO2低于对照组,PaO2明显高于对照组,差异均有统计学意义(P<0.05)。观察组的临床总有效率为92.5%,高于对照组的75.0%,差异有统计学意义(P<0.05)。结论:加味清热化痰汤能有效缓解痰热蕴肺型AECOPD患者的临床症状,通过减少炎性因子的释放来抑制炎症反应,提高临床疗效。

Objective:To investigate the effect of self-made modified Qingre Huatan Decoction on AECOPD of phlegm heat accumulation lung type and its influence on serum inflammatory markers.Method:A total of 80 AECOPD patients who received treatment in our hospital from December 2018 to January 2020 were selected as study subjects.According to the random number table method,they were divided into observation group and control group,40 cases in each group.The control group was treated with conventional western medicine standard treatment,while the observation group was treated with modified Qingre Huatan Decoction on the basis of the control group.The serum inflammatory indexes and blood gas analysis indexes of the two groups before and after treatment were compared,and the clinical efficacy of the two groups was observed.Result:After treatment,the levels of PCT and CRP in the two groups decreased compared with those before treatment,and the levels of PCT and CRP in the observation group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,PaCO2 of the observation group was lower than that of the control group,and PaO2 was significantly higher than that of the control group,the difference were statistically significant(P<0.05).The clinical total effective rate of the observation group was 92.5%,higher than 75.0%of the control group,the difference was statistically significant(P<0.05).Conclusion:Modified Qingre Huatan Decoction can effectively relieve the clinical symptoms of AECOPD patients with phlegm heat accumulation of lung type,inhibit inflammatory reaction by reducing the release of inflammatory factors,and improve the clinical efficacy.
