经皮椎体后凸成形术与经皮椎体成形术治疗压缩性骨折效果及对并发症影响

Effect of Percutaneous Kyphoplasty and Percutaneous Vertebroplasty on Compressive Fractures and Its Effect on Complications

目的:探讨经皮椎体后凸成形术与经皮椎体成形术治疗压缩性骨折效果及对并发症影响。方法:选取2017年1月-2019年1月本院收治的92例脊柱压缩性骨折患者,随机分为对照组和研究组,每组46例。对照组行经皮椎体后凸成形术治疗,研究组行经皮椎体成形术治疗。比较两组患者VAS评分、活动能力评分、手术结果、伤椎后凸Cobb角、椎体中线及前缘高度、并发症发生率。结果:研究组VAS及活动能力评分均低于对照组(P<0.05);研究组骨水泥注入量、手术时间、伤椎丢失及增加高度均低于对照组(P<0.05);研究组伤椎后凸Cobb角小于对照组,椎体中线及前缘高度均高于对照组(P<0.05);研究组并发症发生率低于对照组(P<0.05)。结论:经皮椎体成形术在脊柱压缩性骨折患者临床治疗中发挥重要作用,可降低VAS及活动能力评分,降低伤椎后凸Cobb角,减少骨水泥注入量及伤椎丢失与增加高度,缩短手术时间,可降低并发症发生率,提高治疗安全性。所以,在压缩性骨折患者临床治疗中,经皮椎体成形术应进一步推广和应用。

Objective:To investigate the effects of percutaneous kyphoplasty and percutaneous vertebroplasty in the treatment of compressive fractures and their effects on complications.Method:A total of 92 patients with spinal compression fractures admitted to our hospital from January 2017 to January 2019 were randomly divided into the control group and study group,46 cases in each group.The control group received percutaneous kyphoplasty and the study group received percutaneous vertebroplasty.The VAS score,mobility score,surgical results,Cobb angle of kyphosis,midline and anterior height of vertebral body,and incidence of complications of two groups were compared.Result:The scores of VAS and mobility of the study group were lower than those of the control group(P<0.05).The amount of bone cement injection,operation time,loss and height of the injured vertebrae in the study group were lower than those of the control group(P<0.05).The Cobb angle of kyphosis in the study group was smaller than that of the control group,and the midline and anterior height of the vertebral body were higher than those in the control group(P<0.05).The incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion:Percutaneous vertebroplasty plays an important role in the clinical treatment of patients with vertebral compression fractures,which can reduce VAS and mobility scores,reduce the Cobb angle of injured kyphosis,reduce the amount of bone cement injection,and reduce the loss and increase the height of injured vertebrae.The operation time can reduce the incidence of complications and improve the safety of treatment.Therefore,in the clinical treatment of patients with compression fractures,percutaneous vertebroplasty should be further promoted and applied.
