肝癌患者临床诊断中肝脏增强CT的应用准确率分析

Application Accuracy Analysis of Liver Enhanced CT in Clinical Diagnosis of Liver Cancer

目的:分析肝癌患者临床诊断中肝脏增强CT的应用准确率。方法:选取2016年9月-2019年1月本院收治的肝癌患者108例。根据诊断方法的不同分为对照组和研究组,每组54例。病理诊断作为金标准,对照组应用MRI平扫诊断,研究组应用肝脏增强CT诊断。比较两种检查方法对肝癌的诊断结果、影像学诊断符合情况。结果:病理诊断结果显示对照组中≤3 cm的有20例,>3 cm的有34例,肝脏内病灶30例,肝外病灶24例;研究组中≤3 cm的有22例,>3 cm的有32例,肝脏内病灶28例,肝外病灶26例。研究组对≤3 cm、>3 cm、肝脏内病灶、肝外病灶的判定准确率均明显高于对照组,差异均有统计学意义(P<0.05)。病理诊断结果显示对照组中混合型肝癌有19例,胆管细胞性肝癌有13例,肝细胞性肝癌有22例;研究组混合型肝癌有14例,胆管细胞性肝癌有13例,肝细胞性肝癌有27例。研究组混合型肝癌、肝细胞性肝癌符合率及总符合率均高于对照组,差异均有统计学意义(P<0.05)。结论:在肝癌患者临床诊断中,肝脏增强CT的应用准确率较高,可以为患者治疗方案的制定提供科学依据,临床上应当进一步推广应用。

Objective:To analyze the application accuracy of liver enhanced CT in the clinical diagnosis of liver cancer patients.Method:A total of 108 patients with liver cancer admitted to our hospital from September 2016 to January 2019 were selected.According to different diagnostic methods,the patients were divided into control group and study group,54 patients in each group.Pathological diagnosis was the gold standard,MRI plain scan was used in the control group,and liver enhanced CT was used in the study group.The results of liver cancer diagnosis and imaging diagnosis by the two methods were compared.Result:The pathological diagnosis results showed that in the control group,there were 20 cases ≤3 cm,34 cases with > 3 cm,30 cases of intrahepatic lesions,and 24 cases of extrahepatic lesions.In the study group,there were 22 cases with ≤3 cm,32 cases with > 3 cm,28 cases with intrahepatic lesions and 26 cases with extrahepatic lesions.The accuracy of the study group for the determination of ≤3 cm,> 3 cm,intrahepatic lesions and extrahepatic lesions were significantly higher than those of the control group,the differences were statistically significant (P<0.05).Pathological diagnosis showed that there were 19 cases of mixed liver cancer,13 cases of cholangiocarcinoma and 22 cases of hepatocellular carcinoma in the control group.14 cases of mixed liver cancer,13 cases of cholangiocarcinoma and 27 cases of hepatocellular carcinoma in the study group.The coincidence rate and total coincidence rate of mixed hepatocellular carcinoma and hepatocellular carcinoma in the study group were higher than those in the control group,the differences were statistically significant (P<0.05).Conclusion:In the clinical diagnosis of liver cancer patients,the application accuracy of liver enhanced CT is high,which can provide scientific basis for the formulation of treatment plan for patients,and should be further promoted in clinical application.
