左卡尼汀联合血液透析治疗尿毒症性周围神经病变患者的临床疗效

Clinical Efficacy of L-carnitine Combined with Hemodialysis in the Treatment of Patients with Uremic Peripheral Neuropathy

目的分析左卡尼汀联合血液透析治疗尿毒症性周围神经病变患者的临床疗效。方法本文所选择的研究对象来自2017年6月至2018年10月我院收治的84例尿毒症性周围神经病变患者,将患者随机分为观察组与对照组,每组42例。对照组应用单纯的血液透析方案治疗,观察组则应用左卡尼汀联合血液透析方案治疗。评价并比较两组患者的治疗总有效率、相关临床症状改善情况、神经传导速度。结果观察组患者治疗总有效率(92.86%)显著高于对照组(71.43%),P<0.05。治疗后,观察组患者麻木感、肢端疼痛、不宁腿综合征、感觉障碍比例分别为9.52%、11.90%、11.90%、14.29%,均低于对照组的23.81%、28.57%、21.43%、30.95%,差异具有统计学意义,P<0.05。观察组患者治疗后的胫神经、正中神经和腓总神经传导速度明显比对照组更快,差异存在统计学意义,P<0.05。结论临床治疗尿毒症性周围神经病变患者时,选择在血液透析治疗的基础上联合左卡尼汀进行治疗,改善患者的临床症状,促进患者神经传导速度的提升,提高治疗总有效率。

Objective To analyze the clinical effect of L-carnitine combined with hemodialysis in the treatment of patients with uremic peripheral neuropathy.Methods The research subjects selected in this article were from 84 patients with uremic peripheral neuropathy admitted to our hospital from June 2017 to October 2018.The patients were randomly divided into observation group and control group,with 42 cases in each group.The control group was treated with a pure hemodialysis regimen,and the observation group was treated with L-carnitine combined with hemodialysis regimen.To evaluate and compare the total effective rate of treatment,the improvement of related clinical symptoms,and nerve conduction velocity of the two groups of patients.Results The total effective rate of treatment in the observation group(92.86%)was significantly higher than that in the control group(71.43%),P<0.05.After treatment,the proportions of numbness,limb pain,restless leg syndrome,and sensory disturbance in the observation group were 9.52%,11.90%,11.90%,and 14.29%,respectively,which were lower than of 23.81%,28.57%,21.43%,30.95%in the control group,the difference was statistically significant,P<0.05.The conduction velocity of the tibial nerve,median nerve and common peroneal nerve of the observation group were significantly faster than that of the control group,and the difference was statistically significant,P<0.05.Conclusion When clinically treating patients with uremic peripheral neuropathy,choose to combine L-carnitine on the basis of hemodialysis treatment to improve the patient's clinical symptoms,promote the increase of the patient's nerve conduction velocity,and increase the total effective rate of treatment.
