摘要
目的探讨前列地尔治疗次大面积肺栓塞合并肺动脉高压的临床效果。方法方便选取2017年6月—2019年6月该院收治的82例次大面积肺栓塞合并肺动脉高压患者,采用红蓝双色球法将其分成对照组和观察组,各41例。对照组采用常规抗凝治疗,观察组在此基础上采用前列地尔治疗,14 d后比较两组患者治疗前后的肺动脉压、肺功能变化情况和疗效。结果治疗前观察组的肺动脉压(55.76±9.63)mmHg与对照组的(55.82±9.54)mmHg比较差异无统计学意义(t=0.028,P=0.489>0.05);治疗后观察组的肺动脉压(43.85±4.49)mmHg低于对照组的(51.62±4.57)mmHg(t=7.766,P<0.05);治疗前两组的第1秒用力呼气容积(FEV1)、FEV1与用力肺活量(FVC)的比值(FEV1/FVC)对比差异无统计学意义(t=1.248、0.216,P=0.108、0.415);治疗后观察组的FEV1(1.74±0.17)L、FEV1/FVC(75.54±7.83)%高于对照组的(1.53±0.12)L、(68.35±8.41)%(t=6.462、4.007,P<0.05);观察组总有效率(90.24%)高于对照组(68.29%)(χ^2=6.010,P=0.014)。结论前列地尔治疗次大面积肺栓塞合并肺动脉高压能够有效降低肺动脉压,改善肺功能,提升疗效。
Objective To investigate the clinical effect of alprostadil in the treatment of sub-large pulmonary embolism with pulmonary hypertension.Methods A total of 82 patients with Sub-large Area pulmonary embolism and pulmonary hypertension in the hospital from June 2017 to June 2019 were convenient selected.They were divided into a control group and an observation group with 41 cases each by the red and blue double-color ball method.The control group was treated with conventional anticoagulation therapy,and the observation group was based on this.After 14 d of diltiazem treatment,the pulmonary artery pressure,pulmonary function changes and curative effects of the two groups were compared before and after treatment.Results The pulmonary artery pressure(55.76±9.63)mmHg of the observation group before treatment was not significantly different from that of the control group(55.82±9.54)mmHg(t=0.028,P=0.489>0.05);the pulmonary artery pressure of the observation group after treatment(43.85±4.49)mmHg was lower than(51.62±4.57)mmHg in the control group(t=7.766,P<0.05);the forced expiratory volume in the first second(FEV1),FEV1 and the ratio of forced vital capacity(FVC)(FEV1/FVC)was not significantly different(t=1.248,0.216,P=0.108,0.415);FEV1(1.74±0.17)L,FEV1/FVC(75.54±7.83)%of the observation group after treatment was higher than the control group(1.53±0.12)L,(68.35±8.41)%(t=6.462,4.007,P<0.05);the total effective rate of the observation group(90.24%)was higher than the control group(68.29%)(χ^2=6.010,P=0.014).Conclusion Alprostadil can effectively reduce pulmonary artery pressure,improve pulmonary function,and enhance curative effect in treating sub-large area pulmonary embolism with pulmonary hypertension.
