硝苯地平对妊娠期高血压疾病血压控制效果及对孕妇、胎儿血流动力学状态的影响研究

Effect of Nifedipine on Blood Pressure Control in Hypertensive Disorder of Pregnancy and Its Effect on Hemodynamic Status of Pregnant Women and Fetuses

目的:探讨硝苯地平对妊娠期高血压疾病孕妇血压控制及对孕妇、胎儿血流动力学状态的影响。方法:选取2017年6月-2019年12月本院收治的96例妊娠期高血压疾病孕妇为研究对象,按治疗方式不同将其分为对照组与观察组,各48例。对照组进行硫酸镁治疗,观察组在对照组基础上联合硝苯地平治疗。比较两组血压控制效果、治疗前后血压水平、孕妇(子宫动脉)及胎儿(脐动脉)血流动力学指标。结果:观察组治疗3、7 d后血压控制总有效率均高于对照组(P<0.05)。治疗3、7 d后,观察组收缩压、舒张压、孕妇及胎儿阻力指数(RI)、血流搏动指数(PI)及收缩期与舒张期流速比(S/D)均低于对照组(P<0.05)。结论:硝苯地平对妊娠期高血压疾病孕妇血压控制效果较好,可有效改善孕妇及胎儿血流动力学状态,临床应用价值较高。

Objective:To investigate the effect of Nifedipine on blood pressure control in hypertensive disorder of pregnancy and its effect on hemodynamic status of pregnant women and fetuses.Method:A total of 96 pregnant women with hypertensive disease of pregnancy admitted to our hospital from June 2017 to December 2019 were selected as study subjects.They were divided into control group and observation group according to different treatment methods,48 cases in each group.The control group was treated with Magnesium Sulfate,and the observation group was treated with Nifedipine on the basis of the control group.Blood pressure control effect,blood pressure level before and after treatment,hemodynamic indexes of pregnant women(uterine artery)and fetuses(umbilical artery)were compared between the two groups.Result:The total effective rate of blood pressure control at 3 and 7 d after treatment in the observation group were higher than those in the control group(P<0.05).At 3 and 7 d after treatment,systolic blood pressure,diastolic blood pressure,maternal and fetal resistance index(RI),blood flow pulse index(PI),and ratio of systolic to diastolic velocity(S/D)in the observation group were lower than those in the control group(P<0.05).Conclusion:Nifedipine has a good effect on the control of blood pressure in pregnant women with hypertensive disease of pregnancy,and can effectively improve the hemodynamic status of pregnant women and fetuses,it has a high clinical application value.
