靶向锚定式椎间孔一次成形技术在腰椎间孔镜中的应用

Application of Targeted Anchoring Intervertebral Foramen Once Forming Technique in Lumbar Intervertebral Endoscope

目的:探讨靶向锚定式椎间孔一次成形技术在腰椎间孔镜治疗腰椎间盘突出症中的应用效果。方法:选取2019年1月-2020年1月于本院治疗的50例腰椎间盘突出症患者,依据随机数字表法将其分为对照组与治疗组,各25例。两组均行经皮腰椎间孔镜手术,术中对照组采用常规逐级环踞椎间孔成形技术,治疗组采用靶向锚定式椎间孔一次成形技术。比较两组临床相关指标、Oswestry功能障碍指数(ODI)评分、满意情况及并发症。结果:两组术后8周ODI评分比较,差异无统计学意义(P>0.05)。治疗组椎间孔成形时间与手术时间均短于对照组,视觉模拟评分法(VAS)评分与有效透视次数均低于对照组(P<0.05);治疗组治疗总满意率高于对照组(P<0.05);两组均仅出现神经根损伤1例。结论:靶向锚定式椎间孔一次成形技术经皮腰椎间孔镜手术中取得良好效果,能够简化操作步骤,缩短手术时间与椎间孔成形时间,减少X线暴露次数,减轻患者疼痛程度,提高患者手术满意度,临床应用安全可靠。

Objective:To investigate the effect of targeted anchoring intervertebral foramen once forming technique in the treatment of lumbar disc herniation by lumbar intervertebral endoscope.Method:A total of 50 patients with lumbar disc herniation treated in our hospital from January 2019 to January 2020 were selected.They were divided into control group and treatment group according to the random number table method,with 25 patients in each group.Both groups underwent percutaneous lumbar transforaminal endoscopic surgery.During the operation,the control group adopted the conventional step by step circular intervertebral foramina shaping technique,while the treatment group adopted the targeted anchoring intervertebral foramen once forming technique.Clinical indicators,Oswestry disability index(ODI)score,satisfaction and complications were compared between the two groups.Result:There was no significant difference in ODI score between the two groups at 8 weeks after operation(P>0.05).The forming time of intervertebral foramina and operation time in the treatment group were lower than those in the control group,visual analogue scale(VAS)score and the number of effective fluoroscopy were lower than those in the control group(P<0.05).The total satisfaction rate of treatment in the treatment group was higher than that in the control group(P<0.05).There was only 1 case of nerve root injury in both groups.Conclusion:Targeted anchoring intervertebral foramen once forming technique has achieved good results in percutaneous lumbar.It can simplify the operation steps,shorten the time of operation and foramina forming,reduce the frequency of X-ray exposure,reduce the pain degree of patients,and improve the satisfaction of patients.The clinical application is safe and reliable.
