四妙勇安汤加味治疗葡萄膜炎的临床探讨

Clinical study on modified Simiao Yong’an decoction in treating uveitis

本文通过对治疗热毒炽盛之脱疽专方四妙勇安汤溯源、方解、现代药理及抗炎机制研究,充分论证了四妙勇安汤具有解毒活血通络的功效;葡萄膜炎是一种血管炎,相当于中医的络病,其病因是先天阳性体质与后天热邪相搏,形成热毒,腐络伤血,在病理上谓络病瘀滞,气血失运,形成血瘀、出血、水肿、渗出。周婉瑜主任以四妙勇安汤加味诊治葡萄膜炎的临床验案,在临床疗效、缩短病程、减少复发等方面效果显著,论证了从络病治疗葡萄膜炎的经验,体现中医学异病同治的治疗法则,可以将其作为治疗葡萄膜炎基本方的思路加以推广。

In this paper,through the research on the origin,prescription solution,modern pharmacology and anti-inflammatory mechanism of Simiao Yong’an decoction,a special prescription for removing gangrene used to treat excessive heat toxin,it was fully demonstrated that Simiao Yong’an decoction had the effects of detoxifying,promoting blood circulation and dredging collaterals.Uveitis is a kind of vasculitis,which is equivalent to collateral diseases in Traditional Chinese Medicine.Its etiology is that congenital positive constitution and acquired heat pathogenic factors compete with each other to form heat toxin and rot collaterals to injure blood.Pathologically,it is deemed as collaterals stasis,qi and blood loss,blood stasis,hemorrhage,edema and exudation.Director Zhou Wanyu's clinical trial of treating uveitis with modified Simiao Yong’an decoction achieved remarkable results in clinical curative effect,shortening the course of disease,reducing recurrence and other aspects.She demonstrated the experience of treating uveitis from collateral diseases,embodied the treatment principle of same treatment for different diseases in Traditional Chinese Medicine,that should be promoted as the basic formula for treating uveitis.
