医院、社区协调共管模式对老年高血压管理效果的评价

Evaluation of the Coordination and Co-management Model of Hospital and Community on the Management Effect of Elderly Hypertension

目的该文主要针对医院、社区协调共管模式在老年高血压管理中的治疗成果进行评价,并最终通过讨论总结出适合医院、社区的最佳管理模式。方法2018年5月—2019年5月间该次研究主要从某社区筛选符合标准的老年高血压患者共计260例,按照随机分组的方式,划分对照组及观察组各130例。在对照组中,主要采取的是社区医院护理人员常规性的管理模式;而在观察组中,主要采取的是医院、社区两者相互合作的管理模式,结合医院所具备的医疗技术以及社区的资源整合优势对老年患者高血压疾病实施管理。其次,还要将两组患者的规范管理率、血压控制率以及平均收缩压力、平均舒张压、BMI与杜氏高血压进行生活质量的评估。结果在规范管理率、血压控制率方面进行干预后,观察组的管理效果要高于对照组,差异有统计学意义(P<0.05)。另外,在高血压生活质量评分方面,对于两组在体质指数、平均收缩压及平均舒张压的比基线值等相关干预效果显示,观察组优于对照组,差异有统计学意义(P<0.001)。结论医院、社区协调共管模式能明显提高社区高血压管理、防治水平。

Objective This article mainly evaluates the treatment results of the hospital and community coordination and co-management model in the management of elderly hypertension,and finally summarizes the best management model suitable for the hospital and the community through discussion.Methods From May 2018 to May 2019,in this study,a total of 260 elderly hypertensive patients meeting the criteria were selected from a certain community.According to the method of random grouping,130 cases were divided into the control group and the observation group.In the control group,the routine management mode of nursing staff in community hospitals was mainly adopted;while in the observation group,the management mode was mainly adopted in cooperation between the hospital and the community,combining the medical technology possessed by the hospital and the community’s prevention mode.The advantages of resource integration implement management of hypertension in elderly patients.Secondly,the standard management rate,blood pressure control rate,average systolic pressure,average diastolic pressure,BMI and Dushi hypertension of the two groups of patients should be evaluated for the quality of life.Results After intervention in the standardized management rate and blood pressure control rate,the management effect of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).In addition,in terms of the quality of life scores for hypertension,the observation group is better than the control group,and there is a statistically significant difference between the two groups in terms of the system index and the average systolic blood pressure level 1 average diastolic blood pressure compared to the baseline value(P<0.001).Conclusion The hospital and community coordinated co-management model can significantly improve the management and prevention of hypertension in the community.
