彩色多普勒超声对卵巢囊肿与宫外孕的诊断鉴别作用

Color Doppler Ultrasound in the Diagnosis and Differentiation of Ovarian Cyst and Ectopic Pregnancy

目的将彩色多普勒超声用于卵巢囊肿与宫外孕诊断中,对其鉴别作用进行分析。方法便利选取2018年6月—2019年6月该院收治的25例宫外孕患者为A组,选取同期收治的25例卵巢囊肿患者为B组,两组均行彩色多普勒超声诊断,对两组诊断结果进行分析。结果 A组超声显示复杂型23例,妊娠囊型2例;B组超声显示低回声型11例,薄壁囊肿型4例,厚壁囊肿型7例,薄壁囊肿内部回声型3例;A组PI、RI、S/D,PSV各项血流参数分别为(0.95±0.36)、(0.58±0.23)、(2.53±0.69),(12.15±2.78)cm/s,与B组相比,差异有统计学意义(t=3.205、3.124、3.321、3.306,P<0.05);A组患者CDFI血流频谱多为极低阻力、无舒张期频谱、反向舒张期频谱,B组多为低阻力、高阻力,组间相比差异有统计学意义(P<0.05)。结论卵巢囊肿与宫外孕在包块类型、血流参数、CDFI血流频谱等方面均有一定差异,彩色多普勒超声对宫外孕及卵巢囊肿鉴别作用较好,值得应用。

Objective To use color Doppler ultrasound in the diagnosis of ovarian cysts and ectopic pregnancy, and to analyze their differential effects. Methods 25 patients with ectopic pregnancy admitted from June 2018 to June 2019 were selected as group A, and 25 patients with ovarian cyst admitted at the same time as group B. Both groups were diagnosed by color Doppler ultrasound, and the diagnosis results of the two groups were analyzed. Results Group A ultrasound showed 23 cases of complex type, 2 cases of gestational cystic type;Group B ultrasound showed 11 cases of hypoechoic type, 4 cases of thin-walled cyst type, 7 cases of thick-walled cyst type, and 3 cases of thin-walled internal echo type cyst;The blood flow parameters of PI, RI, S/D and PSV in group A were(0.95 ±0.36),(0.58 ±0.23),(2.53 ±0.69),(12.15 ±2.78)cm/s,respectively. Compared with group B,the difference was statistically significant(t=3.205, 3.124, 3.321, 3.306, P<0.05);CDFI blood flow spectrum of patients in group A was mostly very low resistance, no diastolic spectrum, reverse diastolic spectrum,group B had mostly low resistance, high resistance,the difference was statistically significant between groups(P <0.05).Conclusion Ovarian cysts and ectopic pregnancy have certain differences in mass types, blood flow parameters, CDFI blood flow spectrum, etc. Color Doppler ultrasound has a good effect on distinguishing ectopic pregnancy and ovarian cysts and is worthy of application.
