老年2型糖尿病并发周围神经病变的相关因素研究

Study on Related Factors of Type 2 Diabetes Complicated by Peripheral Neuropathy in the Elderly

目的研究老年2型糖尿病(T2DM)并发周围神经病变(DPN)的相关因素。方法选取该院2018年2月—2019年11月收治的老年T2DM并发DPN患者57例为研究组,取同期老年T2DM患者50例作对照组,行回顾性研究,统计两组临床资料并进行对比,分析与T2DM并发DPN相关因素。结果研究组年龄高于对照组(t=7.573),DM病程长于对照组(t=8.383),腰臀比(1.04±0.04)、SBP (146.31±18.66)mmHg、DBP (90.37±12.90)mmHg、FBG (11.39±2.76)mmol/L、HbA1c (9.80±2.04)%、TG (1.93±0.32)mmol/L、TC (6.63±2.18)mmol/L大于对照组[(0.95±0.07)、(138.06±19.24) mmHg、(81.17±10.59)mmHg、(8.92±2.65)mmol/L、(7.10±1.84)%、(1.34±0.29)mmol/L、(4.91±1.80)mmol/L],差异有统计学意义(t=8.289、2.249、3.997、4.705、7.149、9.939、4.413,P<0.05);经Logistic分析,年龄、病程、HbA1c、FBG均为老年T2DM发生DPN独立危险因素(P<0.05)。结论年龄、病程、HbA1c、FBG升高可增加老年T2DM发生DPN几率,以此制定早期干预措施有助于减少DPN发生。

Objective To study the related factors of elderly type 2 diabetes(T2 DM) with peripheral neuropathy(DPN).Methods 57 elderly T2 DM patients with DPN admitted to the hospital from February 2018 to November 2019 were selected as the study group, and 50 elderly T2 DM patients during the same period were selected as the control group. A retrospective study was performed. The clinical data of the two groups were counted and compared. Analyze the factors related to T2 DM concurrent DPN. Results The study group was older than the control group(t=7.573), the course of DM was longer than the control group(t=8.383), waist-to-hip ratio(1.04±0.04), SBP(146.31±18.66) mmHg, DBP(90.37±12.90) mmHg, FBG(11.39±2.76) mmol/L, HbA1 c(9.80±2.04)%, TG(1.93±0.32) mmol/L, TC(6.63±2.18) mmol/L were greater than the control group [(0.95 ±0.07),(138.06 ±19.24) mmHg,(81.17 ±10.59)mmHg,(8.92 ±2.65)mmol/L,(7.10 ±1.84)%,(1.34±0.29)mmol/L,(4.91±1.80)mmol/L],the difference was statistically significant(t=8.289,2.249,3.997,4.705,7.149,9.939,4.413,P <0.05);after Logistic analysis, age, course of disease, HbA1 c, FBG were all independent risk factors for DPN in elderly T2 DM(P<0.05). Conclusion Age, course of disease, elevated HbA1 c, and FBG can increase the probability of DPN in elderly T2 DM. Early intervention measures can help reduce the occurrence of DPN.
