探讨多层螺旋CT在肋骨骨折诊断中的应用价值 被引量:2

摘要 目的对比并分析肋骨骨折患者采用多层螺旋CT重建扫描与X线摄片的诊断效果与价值。方法随机纳入该院于2017年9月—2019年11月收治60例胸部外伤患者,纳入患者均接受X线摄片检查与多层螺旋CT三维重建扫描,结合临床观察与复诊情况,对比分析不同方法诊断效果。结果在对肋骨骨折诊断的符合率上,螺旋CT重建为98.18%明显高于X线摄片的83.64%,差异有统计学意义(χ^2=6.944,P<0.05);多层螺旋CT三维重建扫描诊断特异度100.00%、敏感度98.18%与准确率98.33%;X线摄片诊断特异度60.00%、敏感度83.64%与准确率81.67%;多层螺旋CT三维重建扫描诊断特异度、敏感度以及诊断准确性相比X线摄片诊断结果更高,差异有统计学意义(χ^2=18.764、7.674、8.326,P<0.05)。结论肋骨骨折采取螺旋CT重建扫描对于骨折的诊断效果更明确,该方法在胸部外伤疑似为肋骨骨折的临床诊断中具有推广应用价值。 Objective To compare and analyze the diagnostic effect and value of multi-slice spiral CT reconstruction scan and X-ray radiography in patients with rib fractures. Methods Randomly enrolled 60 patients with thoracic trauma in the hospital from September 2017 to November 2019. All included patients underwent X-ray radiography and three-dimensional reconstruction scan of multi-slice spiral CT, combined with clinical observation and follow-up,comparative analysis Different methods of diagnosis effect. Results In terms of the coincidence rate for the diagnosis of rib fractures, the 98.18% of spiral CT reconstruction was significantly higher than the 83.64% of X-ray radiographs,The difference was statistically significant(χ^2=6.944, P <0.05);three-dimensional reconstruction of multi-slice spiral CT scans diagnostic specificity was 100.00%, the sensitivity was 98.18% and the accuracy was 98.33%;the X-ray radiography diagnostic specificity was 60.00%, the sensitivity was 83.64%, and the accuracy was 81.67%;the diagnostic specificity, sensitivity, accuracy were higher than that of X-ray radiography and the difference was statistically significant(χ^2=18.764,7.67,4,8.326, P<0.05). Conclusion Rib fractures can be more clearly diagnosed by spiral CT reconstruction scan. This method has the value of popularization and application in the clinical diagnosis of rib fractures suspected of chest trauma.
