超声乳化术在治疗闭角型青光眼合并白内障患者的疗效分析

Analysis of the Effect of Phacoemulsification in the Treatment of Angle-closure Glaucoma Complicated with Cataract

目的观察超声乳化术治疗闭角型青光眼(ACG)合并白内障患者的临床疗效。方法选择2015年7月—2019年12月该院收治的ACG合并白内障患者90例,按随机数字表法将其分为两组,均45例。对照组行白内障囊外摘除术治疗,观察组采用白内障超声乳化术治疗。对比两组临床相关指标[500μm房角开放距离(AOD500)、中央前房深度(ACD)、最佳矫正视力(BCVA)及眼压]及并发症。结果术前两组AOD500、ACD、BCVA及眼压对比,差异无统计学意义(P>0.05);术后观察组AOD500(0.73±0.16)mm、ACD(3.27±0.50)mm高于对照组,BCVA(0.48±0.13)、眼压(15.79±4.18)mmHg低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为6.67%,低于对照组的22.22%,差异有统计学意义(χ2=4.406,P<0.05)。结论超声乳化术是治疗ACG合并白内障的一种安全有效术式,有利于降低患者眼压,改善视力及前房深度,增宽房角,且术后并发症较少,临床应用安全性较高。

Objective To observe the clinical efficacy of phacoemulsification in the treatment of angle-closure glaucoma(ACG)patients with cataract.Methods A total of 90 ACG patients with cataract admitted to the hospital from July 2015 to December 2019 were selected and divided into two groups according to the random number table method,each with 45 cases.The control group was treated with extracapsular cataract extraction,and the observation group was treated with phacoemulsification.The two groups of clinical related indicators[500μm angle opening distance(AOD500),central anterior chamber depth(ACD),best corrected visual acuity(BCVA)and intraocular pressure]and complications were compared.Results There was no statistically significant difference in AOD500,ACD,BCVA and intraocular pressure between the two groups before operation(P>0.05);AOD500(0.73±0.16)mm and ACD(3.27±0.50)mm in the observation group after operation were higher than those in the control group.BCVA(0.48±0.13)and intraocular pressure(15.79±4.18)mmHg were lower than the control group,the difference was statistically significant(P<0.05);the complication rate of the observation group was 6.67%,which was lower than 22.22%of the control group.There was statistical significance(χ2=4.406,P<0.05).Conclusion Phacoemulsification is a safe and effective procedure for the treatment of ACG complicated with cataract.It is beneficial to reduce the intraocular pressure of patients,improve vision and anterior chamber depth,widen the angle of the chamber,and has fewer postoperative complications.The clinical application is safer.
