双侧钢板置入内固定治疗复杂胫骨平台骨折的效果分析

Analysis of the Effect of Bilateral Plate Placement and Internal Fixation in the Treatment of Complex Tibial Plateau Fractures

目的探析复杂胫骨平台骨折患者使用双侧钢板置入内固定治疗的临床应用效果。方法对该院在2017年1月—2019年12月收治的62例复杂胫骨平台骨折患者进行分析,将使用外侧钢板内固定术治疗患者设为对照组(31例),将使用双侧钢板置入内固定术治疗患者设为观察组(31例)。比较两组治疗效果。结果对照组患者手术操作时间(52.7±5.3)min、骨折愈合时间(20.1±3.2)周,相比于观察组的(76.3±5.4)min、(13.9±1.5)周,差异有统计学意义(t=6.732、8.624,P<0.05)。对照组经治疗,效果优良患者21(67.7%),低于观察组28(90.3%),差异有统计学意义(χ2=4.769,P<0.05)。对照组经治疗,术后发生感染、骨外露等并发症6(19.4%),高于观察组的2(6.5%),差异有统计学意义(χ2=5.365,P<0.05)。结论复杂胫骨平台骨折患者给予双侧钢板置入内固定术治疗,有效改善了患者治疗的优良率,其术后并发症明显低于外侧钢板内固定术,临床应用效果也较为理想。

Objective To analyze the clinical application effect of bilateral plate implantation and internal fixation in patients with complex tibial plateau fractures.Methods A total of 62 patients with complex tibial plateau fractures admitted to the hospital from January 2017 to December 2019 were analyzed.The patients treated with lateral plate internal fixation were set as the control group(31 cases),and bilateral plates were treated with internal fixation set as observation group(31 cases).Compare the treatment effects of the two groups.Results The operation time of the control group was(52.7±5.3)min and the fracture healing time was(20.1±3.2)weeks.Compared with the observation group(76.3±5.4)min and(13.9±1.5)weeks,the difference was statistically significant(t=6.732,8.624,P<0.05).After treatment in the control group,21 patients with excellent results(67.7%)were lower than the observation group 28(90.3%)the difference was statistically significant(χ2=2.367,P<0.05).After treatment in the control group,complications such as infection and bone exposure occurred 6(19.4%),which was higher than 2(6.5%)in the observation group,the difference was statistically significant(χ2=5.365,P<0.05).Conclusion Patients with complex tibial plateau fractures were treated with bilateral plate implantation and internal fixation,which effectively improved the good rate of treatment.The postoperative complications were significantly lower than those of lateral plate internal fixation,and the clinical application effect was also satisfactory.
