鼻内窥镜下运用刨削钻行下鼻甲黏膜下部分切除术40例

40 Cases of Partial Submucosal Resection of Inferior Turbinate with Planing Drill under Nasal Endoscope

目的在保留患者鼻甲黏膜生理功能基础上,探究鼻腔内窥镜下使用刨削钻对患者进行下鼻甲黏膜下部分切除术的临床治疗效果。方法该科室将2019年1月—2020年2月来该院接受治疗的80例慢性肥厚性鼻炎患者随机分为两组,行下鼻甲部分切除术患者为对照组(n=40),行下鼻甲黏膜下部分切除术患者为观察组(n=40)。结果观察组经手术治疗,操作时间(15.3±2.6)min、术中出血量(22.1±4.5)mL、创面愈合时间(14.5±2.3)d及术后渗血时间(11.5±2.3)d,各指标组间差异有统计学意义(t=2.215、3.542、4.541、6.452,P<0.05)。观察组经手术治疗,病情好转、显效患者38(95.0%),高于对照组34(85.0%),差异有统计学意义(χ2=8.426,P<0.05)。结论慢性肥厚性鼻炎患者在鼻内窥镜下用刨削钻行鼻甲黏膜下部分切除术,不仅具有手术创口小,创面愈合速度较快等优点,疗效也较为确切,推荐临床使用。

Objective On the basis of preserving the physiological function of the patient's turbinate mucosa,to explore the clinical therapeutic effect of submucosal partial resection of the inferior turbinate using a planer drill under nasal endoscopy.Methods 80 patients with chronic hypertrophic rhinitis who came to the hospital for treatment from January 2019 to February 2020 were randomly divided into two groups by the department.Patients who underwent partial inferior turbinate resection served as the control group(40 cases)and underwent inferior turbinate.Patients with partial submucosal resection were included in the observation group(40 cases).Results The observation group received surgical treatment,the operation time was(15.3±2.6)min,the intraoperative blood loss(22.1±4.5)mL,the wound healing time(14.5±2.3)d,and the postoperative bleeding time(11.5±2.3)d,there were significant differences between the index groups(t=2.215,3.542,4.541,6.452,P<0.05).After surgical treatment,38(95.0%)patients in the observation group got better and markedly effective,which was higher than 34(85.0%)in the control group,the sifference was statistically significant(χ2=8.426,P<0.05).Conclusion In patients with chronic hypertrophic rhinitis,partial submucosal resection of the turbinate with a planing drill under nasal endoscopy has the advantages of small surgical wounds and faster wound healing,and the curative effect is relatively accurate.It is recommended for clinical use.
