摘要
目的探究CKC(冷刀锥切术)与LEEP(宫颈环形电切术)治疗CIN(宫颈上皮内瘤变)Ⅲ级的临床疗效。方法回顾性收集该院2016年12月—2018年12月120例宫颈上皮内瘤变Ⅲ级患者的临床资料,以治疗方式为依据,分为两组,甲组行LEEP术,乙组行CKC术,对比两组手术指标、HPV(人乳头瘤病毒)持续感染率、疾病复发率。结果甲组手术时间为(8.15±1.21)min,短于乙组,术中出血量为(10.25±0.28)mL,少于乙组,差异有统计学意义(t=46.481、128.210,P<0.05);乙组术后疾病总复发率为5.00%,明显低于甲组的20.00%,差异有统计学意义(χ2=6.171,P<0.05);乙组术后HPV感染率为3.33%,明显低于甲组的20.00%,差异有统计学意义(χ2=8.086,P<0.05)。结论CKC术与LEEP术均能有效治疗CINⅢ级,LEEP术手术耗时短,术中出血量少,CKC术疾病复发率与HPV感染率低,临床可视患者实际情况合理选择手术方式治疗。
Objective To explore the clinical effects of CKC(cold knife conization)and LEEP(circular cervical resection)in the treatment of CIN(cervical intraepithelial neoplasia)grade III.Methods The clinical data of 120 patients with cervical intraepithelial neoplasia grade III in the hospital from December 2016 to December 2018 were retrospectively collected.Based on the treatment method,they were divided into two groups,group A underwent LEEP operation,and group B underwent CKC;to compare the surgical indicators,HPV(human papillomavirus)persistent infection rate,and disease recurrence rate between the two groups.Results The operation time of group A was(8.15±1.21)min,which was shorter than that of group B,and the intraoperative blood loss was(10.25±0.28)mL,which was less than that of group B,the difference was statistically significant(t=46.481,128.210,P<0.05);group B after operation,the total disease recurrence rate was 5.00%,which was significantly lower than 20.00%in group A,the difference was statistically significant(χ2=6.171,P<0.05);the postoperative HPV infection rate in group B was 3.33%,which was significantly lower than 20.00%in group A,the difference was statistically significant(χ2=8.086,P<0.05).Conclusion Both CKC and LEEP can effectively treat CIN grade III.LEEP takes a short time and has less blood loss during the operation.The recurrence rate and HPV infection rate of CKC operation are low.The surgical treatment can be reasonably selected according to the actual situation of the patient.
