阿尔茨海默症患者局部脑代谢异常与淡漠的相关性析:一项波谱磁共振研究 被引量:2

摘要 目的:使用质子磁共振波谱(1H-MRS)研究了与AD患者的淡漠相关的额叶神经化学递质改变情况。方法:选择2019年1月—2020年6月期间在我院诊断为阿尔茨海默病(AD)痴呆患者33人,采用淡漠评定量表(AES)评定AD患者淡漠程度。用1H-MRS测定扣带回前区和右侧眶额区的神经代谢物水平。分析神经代谢物与AES总分和各维度分值的关系。结果:共33例患者完成研究。在调整了年龄、性别、教育程度、痴呆症严重程度和抑郁严重程度后,扣带前区N-乙酰天冬氨酸/肌酸(NAA)比值较低的患者显示出较高的总体淡漠评分(β=-0.56,P=0.003)。在进一步的分析中,扣带回前区较低的NAA/Cr与淡漠的所有维度相关,包括认知(β=-0.43,P=0.028)、行为(β=-0.55,P=0.002)和情绪(β=-0.5,P=0.005)。而在右眼眶额区没有发现有统计学意义的关联。结论:我们的结果表明,AD患者中淡漠的认知、行为和情绪维度与前扣带回区域的脑神经代谢物改变有关。前扣带回皮质神经元完整性的异常可能在导致AD患者各个维度的淡漠。
