子宫肌瘤社会心理影响因素的结构方程模型分析

Structural Equation Model Analysis of Social and Psychological Factors of Uterine Fibroids

目的探讨常见社会心理因素对子宫肌瘤发生的影响及作用机制。方法采用问卷调查量表对319名子宫肌瘤病人和319名健康对照进行调查。结果两组研究对象的焦虑、抑郁、D型人格、社会支持的各维度得分差异均有统计学意义(P<0.05)。对焦虑、抑郁、D型人格、社会支持各变量维度得分进行Pearson相关性分析,结果显示,各研究变量两两之间均有统计学关联(P<0.001)。结构方程模型分析结果表明,抑郁对子宫肌瘤具有直接效应(影响系数为0.229),焦虑对子宫肌瘤既有直接效应(影响系数为0.312)又有间接效应(影响系数为0.134),而D型人格和社会支持则通过抑郁、焦虑对子宫肌瘤产生间接效应。结论D型人格及社会支持可能通过抑郁、焦虑间接影响子宫肌瘤的发生,应加强女性的社会心理健康水平,预防子宫肌瘤的发生。

Objective To investigate the effect and mechanism of common social and psychological factors on the occurrence of uterine fibroids.Methods A questionnaire was used to investigate 319 patients with uterine fibroids and 319 healthy controls.Results The difference in the scores of anxiety,depression,Type D personality and social support of the two groups were statistically significant(P<0.05).Pearson correlation analysis was performed on the scores of the four variable dimensions,and the result indicated that there was a statistical correlation between any two variables(P<0.001).The results of structural equation model analysis suggested that depression had direct effect on uterine fibroids(the influence coefficient was 0.229),while anxiety had both direct effect(the influence coefficient was 0.312)and indirect effect(the influence coefficient was 0.134)on uterine fibroids.Type D personality and social support had indirect effect on uterine fibroids through depression and anxiety.Conclusion Type D personality and social support may have indirect effect on the occurrence of uterine fibroids through depression and anxiety,so women's psychosocial health should be strengthened to prevent uterine fibroids.
