高血压合并无症状性急性心肌梗死的临床特征及危险因素分析

目的探讨高血压合并无症状性心肌梗死(HTN-SIM)的临床特征及危险因素分析。方法连续性选择2016年2月至2019年2月就诊于我院符合纳入及排除标准的HTN-SIM患者51例,同期选取符合纳入、排除标准的高血压患者140例,比较其临床特征、分析发生HTN-SIM的危险因素。结果与高血压组患者相比,HTN-SIM组患者年龄更大、高血压病程更长,出现头晕、呼吸困难症状占比更多,有口服ACEI/ARB药物史和患者冠心病、高尿酸血症患者占比更多。logistic多因素回归分析结果显示:高血压病程长(OR=1.092)、冠心病史(OR=3.107)为高血压发展成HTN-SIM的独立危险因素。结论大龄、高血压病程长、并存病多是HTN-SIM的临床特征,头晕、呼吸困难是HTN-SIM常见的临床症状。此外,高血压病程长、冠心病史为高血压发展成HTN-SIM的独立危险因素。

Objective To compare the clinical characteristics and risk factors of hypertension with silent myocardial infarction(HTN⁃SIM).Methods 51 consecutive patients with HTN⁃SIM who were addressed in our hospital from February 2016 to February 2019 were selected continuously,and 140 patients with hypertension(HTN)were selected during the same period.Their clinical characteristics were compared and their risk factors were analyzed.Results Compared with HTN group,HTN⁃SIM group were older and had a longer course of hyper⁃tension.Compared with HTN group,HTN⁃SIM group had more dizziness,more dyspnea,more oral ACEI/ARB drug history,more coronary heart disease,and more hyperuricemia.Logistic regression analysis showed that the long course of hypertension(OR=1.092)and the history of coronary heart disease(OR=3.107)was independent risk factors for HTN⁃SIM.Conclusion Old age,long course of hypertension,and coexisting diseases are the clinical features of HTN⁃SIM.Dizziness and dyspnea are common clinical symptoms of HTN⁃SIM.In addition,we also found that the long course of hypertension and history of coronary heart disease are independent risk factors for the development of hypertension intoHTN⁃SIM.
