全科医生服务模式下2型糖尿病患者三高控制情况分析

Analysis on the Control of the Three Highs of Type 2 Diabetes Patients under the General Practitioner Service Mode

目的分析与探讨在全科医生服务模式下,2型糖尿病患者控制三高(高血压、高血脂、高尿酸)的情况分析。方法选取2018年3月—2019年3月于该中心门诊就诊的168例合并三高的糖尿病患者为研究对象,按照是否接受全科医生服务模式为依据,分为对照组和观察组,每组84例。对照组采用传统服务模式(生物医学模式),观察组采用全科医生服务模式,分析在全科医生服务模式下,合并三高情况的2型糖尿病患者的各项指标变化及用药依从性、医嘱认可度等方面的比较。结果观察组血糖、三高(高血压、高血脂、高尿酸)控制情况分别为FPG(7.28±1.48)mmol/L、2 hPBG(8.83±2.21)mmol/L、HbA1c(6.38±1.27)%;SBP(115.86±20.15)mmHg、TG(2.30±0.28)mmol/L、TC(5.10±0.5)mmol/L各项指标均优于对照组,两组比较差异有统计学意义(P<0.05)。且用药依从性及遵医嘱行为方面均优于对照组,两组比较差异有统计学意义(P<0.05)。结论相比传统服务模式,全科医生服务模式更能有效的改善患者的血糖、三高情况以及患者的遵医行为,效果明显,值得临床推广。

Objective To analyze and discuss the control of the three high levels(hypertension,hyperlipidemia,and hyperuric acid)in patients with type 2 diabetes under the general practitioner service model.Methods A total of 168 diabetic patients with three highs who were treated in the outpatient clinic of the center from March 2018 to March 2019 were selected as the research objects.Based on whether they received general practitioner service mode,they were divided into control group and observation group,each group 84 cases.The control group adopted the traditional service model(biomedical model),and the observation group adopted the general practitioner service model.To analyze the changes of various indicators,medication compliance and medical order recognition of type 2 diabetes patients with three high conditions under the general practitioner service mode.Results The control status of blood glucose and three highs(hypertension,hyperlipidemia,hyperuric acid)in the observation group were FPG(7.28±1.48)mmol/L,2 hPBG(8.83±2.21)mmol/L,HbA1c(6.38±1.27)%;SBP indexes of(115.86±20.15)mmHg,TG(2.30±0.28)mmol/L,and TC(5.1±0.5)mmol/L were better than those of the control group,and the difference between the two groups was statistically significant(P<0.05).And medication compliance and compliance behavior were better than the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion Compared with the traditional service model,the general practitioner service model can effectively improve the patient's blood glucose,three highs,and patients'compliance behavior.The effect is obvious and it is worthy of clinical promotion.
