手术护理路径配合模式对人工髋关节置换术患者术后康复及切口感染率的影响

Effect of surgical nursing path coordination mode on postoperative rehabilitation and incision infection rate of patients undergoing artificial hip replacement

目的:探讨手术护理路径对人工髋关节置换术患者的影响。方法:选取2017年1月至2019年12月实施手术护理路径配合模式的145例行人工髋关节置换术的患者为观察组,另外选取2014年8月至2016年12月实施手术护理路径配合模式前采用常规护理模式的145例行人工髋关节置换术的患者为对照组,比较两组术后1、2、4周髋关节Harris评分,术后1、6、12 h VAS评分,切口愈合及感染情况,手术时间、术中出血量、平均住院时间及骨折愈合时间,患者对护理满意度。结果:术后1、2、4周观察组髋关节Harris评分较对照组均显著提高;术后1、6、12 h观察组VAS较对照组均显著下降;观察组6例切口感染,而对照组19例切口感染,观察组切口感染率为4.14%较对照组13.10%显著下降,且观察组切口愈合等级优于对照组;观察组手术时间、术中出血量较对照组显著减少,平均住院时间、骨折愈合时间较对照组显著缩短;观察组护理满意率为94.48%,较对照组的82.07%显著提高;差异均有统计学意义(P<0.05)。结论:护理路径配合模式应用于人工髋关节置换术中可降低切口感染发生率,缓解疼痛,促进髋关节功能恢复,提高患者对于护理服务的满意度,值得在临床上推广。

Objective:To explore the effect of surgical nursing path coordination mode on postoperative rehabilitation and incision infection rate of patients undergoing artificial hip replacement surgery;Methods:Select 145 routines who implemented surgical nursing path coordination mode from January 2017 to December 2019 The patients undergoing artificial hip replacement were the observation group.In addition,145 patients who underwent the artificial hip replacement surgery before the implementation of the surgical nursing path coordination mode from August 2014 to December 2016 were selected as the control group.In the group 1,2,4 weeks after operation,the Harris score of the hip joint,the VAS score at 1,6,and 12 hours after the operation,the wound healing and infection,the operation time,intraoperative blood loss,average hospital stay and fracture healing time,the patients were satisfied with the nursing care Results:The Harris score of the hip in the observation group was significantly higher than that in the control group at 1,2,and 4 weeks after the operation.The VAS in the observation group was significantly lower than that in the control group at 1,6,and 12 hours after the operation.There were 6 cases of incisional infection in the observation group.In the control group,there were 19 cases of incision infection.The incision infection rate of the observation group was 4.14%,which was significantly lower than that of the control group.The incision healing grade of the observation group was better than that of the control group.The operation time and intraoperative blood loss of the observation group were significantly reduced compared with the control group.Hospitalization time and fracture healing time were significantly shorter than those of the control group.The nursing satisfaction rate of the observation group was 94.48%,which was significantly higher than that of the control group 82.07%.The difference was statistically significant(P<0.05);Conclusion:The nursing path coordination model is applied to artificial hip Joint replacement surgery can reduce the incidence of incision infection,relieve pain,promote the recovery of hip joint function,and improve patient satisfaction with nursing services.It is worthy of clinical promotion.
