大肠癌组织瘦素、骨桥蛋白表达与大肠癌根治术后复发、转移的关系

Relationship between leptin,OPN expression in colorectal cancer tissues and recurrence,metastasis after radical resection

目的探讨大肠癌组织瘦素(leptin)、骨桥蛋白(OPN)表达与大肠癌复发、转移的关系。方法2012年4月-2014年4月在本院接受治疗的大肠癌患者140例,均接受大肠癌根治术+术后标准FOXFLO4方案化疗,采用免疫组化检测leptin、OPN蛋白阳性表达率。对比大肠癌组织、癌旁组织的leptin、OPN阳性率,不同病理特征的大肠癌组织leptin、OPN阳性率,不同术后复发、转移情况患者的病灶组织leptin、OPN阳性率差异。结果大肠癌组织中leptin、OPN表达阳性率高于癌旁组织(P<0.05)。不同病理特征的大肠癌患者病灶组织中leptin、OPN表达阳性率的差异有统计学意义(P<0.05)。其中病灶直径≥4 cm、浸润深度≥5 cm、低分化程度、FIGO分期Ⅱa~Ⅱb期、伴淋巴结转移的大肠癌患者leptin、OPN表达阳性率更高(P<0.05)。3年、5年复发及转移患者的leptin、OPN表达阳性率高于非复发患者,差异有统计学意义(P<0.05)。结论leptin、OPN在大肠癌组织中表达阳性率异常增高,其高表达可能提示病情严重及术后复发转移率较高。

Objective To investigate the relationship between leptin,OPN expression in colorectal cancer tissues and recurrence,metastasis after radical resection.Methods 140 cases of colorectal cancer patients in our hospital from April 2012 to April 2014 were chosen as research subject,they were treated with radical resection of colorectal cancer plus postoperative standard FOXFLO4 chemotherapy.The positive expression rates of leptin,OPN proteins were detected by immunohistochemistry.The positive rates of leptin,OPN in colorectal cancer tissues and adjacent tissues were compared.The positive rates of leptin,OPN in colorectal cancer tissues with different pathological characteristics were compared.The positive rates of leptin,OPN with different postoperative recurrence and metastasis were compared.Results Positive rate of leptin,OPN in colorectal cancer tissues was higher than those in adjacent tissues(P<0.05).Positive rates of leptin,OPN in the lesion tissues of colorectal cancer patients with different pathological characteristics were statistically significant(P<0.05);lesion diameter≥4 cm,infiltration depth≥5 cm,low degree of differentiation,FIGO stagingⅡa-Ⅱb period,leptin with lymph node metastasis of colorectal cancer patients,leptin,OPN expression positive rate were higher(P<0.05).The positive rate of leptin,OPN expression in 3 and 5 year recurrence,metastasis cases were higher than those in non-recurrence cases,with the differences statistical significance(P<0.05).Conclusion Positive rates of leptin,OPN expression in colorectal cancer tissues increase abnormally,and the high expression of leptin,OPN may indicate the severity of disease and the high rate of postoperative recurrence,metastasis.
