摘要
(1)目的观察达格列净对2型糖尿病(T2DM)合并高尿酸血症(HUA)患者血尿酸(SUA)及24小时尿尿酸(UA)水平的影响。(2)方法采用随机对照实验研究,将90例T2DM患者随机分为观察组(给予达格列净联合胰岛素治疗)40例与对照组(单纯给予胰岛素治疗)50例,治疗12周,比较治疗前后患者SUA、24小时尿UA、空腹血糖(FPG)、糖化血红蛋白(HbA1c)水平及胰岛素用量的变化。(3)结果治疗后观察组SUA、FPG、HbA1c水平均较治疗前降低,24小时尿UA排泄较治疗前增加,胰岛素用量较前减少(P<0.05);治疗后对照组SUA、FPG、HbA1c水平均较治疗前降低,24小时UA排泄较前减少,胰岛素用量增加(P<0.05)。治疗后两组间比较,24小时尿UA、HbA1c水平及胰岛素用量差异有统计学意义(P<0.05),SUA、FPG差异无统计学意义(P>0.05)。(4)结论达格列净联合胰岛素治疗T2DM合并高尿酸血症可减少胰岛素用量,降低血糖、血尿酸水平,增加肾脏尿酸排泄。
Objective To observe the effects of daglicaten in the treatment of type 2 diabetes(T2DM)with hyperuricemia(HUA)in patients with serum uric acid(SUA)and 24-hour uric acid(UA).Methods Using a randomized controlled experimental study,90 patients with T2DM admitted to the hospital were randomly divided into observation group(dagliflozin combined with insulin in 40 cases)and control group(50 cases of insulin alone)for 12 weeks of treatment.The SUA of patients before and after treatment was compared,24 hour urine UA,fasting blood glucose(FPG),glycated hemoglobin(HbA1c)and insulin consumption.Results After treatment,the levels of SUA,FPG and HbA1c in the observation group were lower than before treatment.The 24-hour urinary UA excretion was increased compared with before treatment,and the amount of insulin was decreased(P<0.05).After treatment,the levels of SUA,FPG and HbA1c were lower in the control group.Compared with before treatment,UA excretion decreased,and insulin consumption increased(P<0.05).After treatment,the difference between the UA,HbA1c and insulin consumption was statistically significant between the two groups(P<0.05),and there was no significant difference in SUA and FPG(P>0.05).Conclusion The combination of daglitazone and insulin in patients with T2DM and hyperuricemia can reduce insulin consumption,lower blood sugar levels,increase renal uric acid excretion,and lower blood uric acid levels in patients.
