摘要
目的研究系统性红斑狼疮(SLE)患者血清β2-微球蛋白(β2-MG)、25羟维生素D[25(OH)D]水平与肾功能的相关性。方法将我院收治的121例SLE患者根据肾小球滤过率(eGFR)进行分组,eGFR≥90 mL/(min·1.73 m^2)为肾功能正常组(n=30),eGFR 60~<90 mL/(min·1.73 m^2)为肾功能轻度异常组(n=30),eGFR 30~<60 mL/(min·1.73 m^2)为肾功能中度异常组(n=31),eGFR<30 mL/(min·1.73 m^2)为肾功能重度异常组(n=30)。比较四组的血清β2-MG、25(OH)D水平与eGFR,并分析血清β2-MG、25(OH)D水平与eGFR的相关性。结果血清25(OH)D水平、eGFR:肾功能重度异常组<肾功能中度异常组<肾功能轻度异常组<肾功能正常组(P<0.05);血清β2-MG水平:肾功能重度异常组>肾功能中度异常组>肾功能轻度异常组>肾功能正常组(P<0.05)。经双变量Pearson相关性检验显示,eGFR与血清25(OH)D水平呈正相关,与血清β2-MG水平呈负相关(P=0.000)。结论SLE患者的肾功能损伤越严重,血清25(OH)D水平越低,血清β2-MG水平越高,血清β2-MG和25(OH)D水平可作为临床初步诊断SLE患者肾功能的依据。
Objective To study the correlation between the levels of serumβ2-microglobulin(β2-MG),25 hydroxyvitamin D[25(OH)D]and renal function in patients with systemic lupus erythematosus(SLE).Methods A total of 121 SLE patients admitted in our hospital were grouped according to the glomerular filtration rate(eGFR),eGFR≥90 mL/(min·1.73 m^2)were seted as normal renal function group(n=30),eGFR 60~<90 mL/(min·1.73 m^2)were seted as mild abnormal renal function group(n=30),eGFR 30~<60 mL/(min·1.73 m^2)were seted as moderate abnormal renal function group(n=31),eGFR<30 mL/(min·1.73 m^2)were seted as severe abnormal renal function group(n=30).The serumβ2-MG,25(OH)D levels and eGFR were compared among the four groups,and the correlation between serumβ2-MG,25(OH)D levels and eGFR were analyzed.Results Serum 25(OH)D level and eGFR:the severe abnormal renal function group<the moderate abnormal renal function group<the mild abnormal renal function group<the normal renal function group(P<0.05);serumβ2-MG level:the severe abnormal renal function group>the moderate abnormal renal function group>the mild abnormal renal function group>the normal renal function group(P<0.05).Bivariate Pearson correlation test showed that eGFR was positively correlated with serum 25(OH)D level and negatively correlated with serumβ2-MG level(P=0.000).Conclusion The more serious the renal function injury in SLE patients,the lower the serum 25(OH)D level and the higher serumβ2-MG level.The levels of serumβ2-MG and 25(OH)D can be used as the basis for the preliminary diagnosis of renal function in SLE patients.
