地诺前列酮与缩宫素对妊娠期糖尿病孕妇引产临产时间和分娩结局的影响

Effect of dinoprostone and oxytocin on induced labor time and delivery outcome of pregnant women with gestational diabetes mellitus

目的分析地诺前列酮与缩宫素对妊娠期糖尿病(GDM)孕妇引产临产时间和分娩结局的影响。方法选取我院2018年1月至2019年5月收治的84例GDM孕妇作为研究对象,随机分为对照组(42例,缩宫素)和观察组(42例,地诺前列酮)。比较两组孕妇宫颈Bishop评分、用药至临产时间、总产程、母婴结局、分娩方式、引产效果及不良反应发生情况。结果用药后6、8、12 h,两组宫颈Bishop评分均升高,且观察组明显高于对照组(P<0.05)。观察组用药至临产时间及总产程均明显短于对照组(P<0.05)。观察组产后出血量明显低于对照组,新生儿Apgar评分明显高于对照组(P<0.05)。观察组阴道分娩率、引产总有效率均明显高于对照组,不良反应总发生率明显低于对照组(P<0.05)。结论地诺前列酮应用于GDM孕妇的引产效果明显,临产时间及分娩结局均优于缩宫素,引产成功率及安全性高。

Objective To analyze the effect of dinoprostone and oxytocin on induced labor time and delivery outcome of pregnant women with gestational diabetes mellitus(GDM).Methods A total of 84 pregnant women with GDM in our hospital from January 2018 to May 2019 were selected as the research objects and randomly divided into control group(42 cases,oxytocin)and observation group(42 cases,dinoprostone).The cervical Bishop score,the time from medication to parturition,total labor process,maternal and infant outcomes,delivery mode,induced labor effect and adverse reactions were compared between the two groups.Results At 6,8,12 h after treatment,the Bishop score in the two groups increased,and that in the observation group was significantly higher than the control group(P<0.05).The time from medication to parturition and total labor in the observation group were significantly shorter than those in the control group(P<0.05).The amount of postpartum hemorrhage in the observation group was significantly lower than that in the control group,and the neonatal Apgar score was significantly higher than that in the control group(P<0.05).The vaginal delivery rate and total effective rate of induced labor in the observation group were significantly higher than those in the control group,and the total incidence of adverse reactions was significantly lower than that in the control group(P<0.05).Conclusion Dinoprostone for the induced labor of inpregnant women with GDM has obvious effect,its labor time and delivery outcome are better than oxytocin,with high induced labor success rate and safety is high.
