温肾蠲痹方联合来氟米特治疗肾虚督寒证强直性脊柱炎疗效观察

Observation on Therapeutic Effect of Wenshen Juanbi Decoction combined with Leflunomide on Ankylosing Spondylitis of Kidney Deficiency and Du Cold Syndrome

目的观察温肾蠲痹方联合来氟米特治疗肾虚督寒证强直性脊柱炎的疗效。方法将80例肾虚督寒证强直性脊柱炎患者按照随机数字表法分为观察组和对照组各40例。对照组给与来氟米特,10 mg/d,治疗12个月;观察组在对照组基础上加用温肾蠲痹方,2次/d,早晚温服,治疗12个月。观测两组临床疗效、Bath强直性脊柱炎疾病活动性指数(BASDAI)、Bath强直性脊柱炎测量指数(BASMI)、Schober试验、扩胸度、指地距、晨僵时间、关节疼痛个数、红细胞沉降率(ESR)、C反应蛋白(CRP)、不良反应。结果治疗后,临床疗效观察组与对照组具有等效性(P>0.05);治疗后,两组BASDAI、BASMI均降低,观察组低于对照组(P<0.05);两组指地距、晨僵时间、关节疼痛个数、ESR、CRP均降低,观察组低于对照组(P<0.05);两组Schober试验、扩胸度均增加,观察组高于对照组(P<0.05);观察组不良反应低于对照组,差异无统计学意义(P>0.05)。结论温肾蠲痹方联合来氟米特治疗肾虚督寒证强直性脊柱炎可改善临床症状及患者体征,降低炎性反应,不增加不良反应发生率,值得推广。

Objective To observe on therapeutic effect of Wenshen Juanbi decoction combined with leflunomide on ankylosing spondylitis of kidney deficiency and Du cold syndrome.Methods Eighty cases of ankylosing spondylitis with kidney deficiency and Du cold syndrome were divided into observation group and control group according to random number table.The control group was given leflunomide,10 mg/d,for 12 months.The observation group was given Wenshen Juanbi decoction,twice a day,taking in the morning and eveningfor 12 months.The clinical effect,BASDAI,BASMI,Schober test,chest expansion,finger ground distance,morning stiffness time,number of joint pain,ESR,CRP and adverse reactions were observed.Result After treatment,the clinical effect of the observation group was equivalent to that of the control group(P>0.05).After treatment,the BASDAI and BASMI of the two groups were lower than that of the control group(P<0.05).The finger ground distance,the time of morning stiffness,the number of joint pain,ESR and CRP of the two groups were lower than those of the control group(P<0.05).The Schober test and chest expansion of the two groups were increased,and those of the observation group was higher than those of the control group(P<0.05).The adverse reactions in the observation group were lower than those in the control group,with no significant difference(P>0.05).Conclusion Wenshen Juanbi decoction combined with leflunomide in the treatment of ankylosing spondylitis of kidney deficiency and Du cold syndrome can improve the clinical symptoms and patients’physical signs,reduce the inflammatory response,and do not increase the incidence of adverse reactions,which is worthy of promoting.
