骨科手术患者医院感染的影响因素分析及防控对策

Analysis of influencing factors of nosocomial infection in orthopedic surgery patients and its prevention and control measures

目的探讨骨科手术患者医院感染的影响因素及防控对策,为医院相关部门提供积极的参考依据。方法选取本院2016年6月-2019年6月在骨科住院的手术患者1760例作为研究对象,采用回归分析法对骨科手术患者医院感染的影响因素进行分析。结果1760例患者医院感染的发生例数为57例,医院感染率为3.23%。多因素回归分析显示,皮肤损伤、气管切开、免疫功能低下,卧床时间长短、手术切口的类型、是否有基础疾病、是否留置导尿、急诊手术是影响骨科手术患者医院感染的主要因素(P<0.05)。结论通过加强对骨科患者基础疾病的积极治疗、缩短患者的住院时间、严格执行各项无菌操作、尽早拔出留置导管,严格落实各项预防医院感染的措施,是降低骨科手术患者医院感染的重要举措。

Objective To explore the influencing factors and prevention and control measures of nosocomial infection in orthopedic surgery patients,and provide reference for relevant hospital departments.Methods A total of 1760 surgical patients hospitalized in our department from June 2016 to June 2019 were selected as the research subjects.The influencing factors of nosocomial infection in orthopedic surgery patients were analyzed by regression analysis.Results The number of hospital infections in 1760 patients was 57,and the hospital infection rate was 3.23%.Multivariate regression analysis showed skin damage,tracheotomy,immune function,bed length,type of surgical incision,basic diseases,indwelling catheterization and emergency surgery were the main factors affecting nosocomial infection in patients undergoing orthopedic surgery(P<0.05).Conclusion By strengthening the active treatment of underlying diseases in orthopedic patients,shortening the length of hospital stay,strictly implementing various aseptic procedures,removing indwelling catheters as soon as possible,and strictly implementing measures to prevent nosocomial infections,it is important to reduce nosocomial infections in orthopedic surgery patients.
