陈永灿治疗慢性萎缩性胃炎用药特色挖掘研究

Data Mining of CHEN Yongcan’s Medication Characteristics In Treatment of Chronic Atrophic Gastritis

目的基于古今医案云平台V1.5软件,整理总结陈永灿治疗慢性萎缩性胃炎的用药特色。方法收集2015年1月—2018年12月就诊于浙江省立同德医院陈永灿治疗慢性萎缩性胃炎医案处方,采用频次、关联及聚类分析。结果共收集处方206首,中药232味,性味以温平寒、甘苦辛为多,归经以脾胃肺肝为主;功效统计显示,常见治法为清热解毒、燥湿化痰、疏肝和胃、补脾益气;高频药物有甘草、陈皮、大枣、白术、半夏、白芍、玫瑰花等;常用药物组合有"甘草-大枣""甘草-陈皮"等;关联规则显示有"黄连→吴茱萸"等;系统聚类提取共获得4个核心聚类群。结论陈永灿治疗慢性萎缩性胃炎,以仲景顾护脾胃为宗旨,以健脾气、理滞气、化痰湿、清郁热为主要治法,善用四君子汤、二陈汤、百合汤、左金丸、小陷胸汤、枳术丸、芍药甘草汤等简易名方灵活辨证,并独创经验方三花百草饮芳香悦神、理气化滞以截断病情、防止传变。

Objective To summarize the medication characteristics of CHEN Yongcan treating chronic atrophic gastritis(CAG)by ancient and modern medical records cloud platform V1.5 software.Methods The information of CAG patients treated by CHEN Yongcan in Tongde Hospital of Zhejiang Province from January 2015 to December 2018 was collected.The frequency analysis,cluster analysis and correlation analysis were carried out.Results A total of 206 prescriptions were obtained,involving 232 kinds of Chinese materia medica.The main medicine tastes were bitter,sweet and spicy,and the main medicine properties were warm,cold and neutral.Medicine efficacy was mainly in spleen,stomach,lung and liver meridians.The efficacy statistics showed the common therapies were clearing heat and detoxifying,drying dampness and resolving phlegm,soothing liver and harmonizing stomach,invigorating spleen and supplementing Qi.Medicines with high frequency were Gancao(liquorice),Chenpi(dried tangerine),Dazao(jujube),Baizhu(macrocephala),Banxia(pinellia ternata),Baishao(radix paeoniae alba)and Meiguihua(rose).The common medicine pairs were"Gancao(liquorice)-Dazao(jujube)""Gancao(liquorice)-Chenpi(dried tangerine)"and so on.Association rule displayed"Huanglian(Coptis chinensis)-Wuzhuyu(fructus evodiae)"and so on.Four core clusters were obtained by system cluster extraction.Conclusion For the purpose of protecting spleen and stomach in the treatment of CAG,CHEN Yongcan takes the methods of invigorating spleen and supplementing Qi,promoting circulation of Qi,resolving phlegm and dampness,and clearing heat as the main therapies.Mr.CHEN used Sijunzi Decoction,Erchen Decoction,Baihe Decoction,Zuojin Pill,Xiaoxianxiong Decoction,Zhizhu Pill,Shaoyao Gancao Decoction and so on.Mr.CHEN has created empirical formula"Sanhua Baicao Yin"to calm the mind,regulate Qi and resolve the stagnation in order to stop and prevent the progress of the disease.
