改良大隐静脉剥脱术联合泡沫硬化剂治疗下肢静脉曲张的效果观察

目的观察改良大隐静脉剥脱术联合泡沫硬化剂治疗下肢静脉曲张的效果。方法选取2016年1月至2018年1月下肢静脉曲张患者78例,根据随机数字表法分为两组,其中40例为传统组,采用传统手术方式,38例为改良组,采用改良大隐静脉剥脱术联合泡沫硬化剂治疗,比较两组的围术期情况和治疗效果。结果所有患者均成功完成手术,无严重并发症发生。两组患者术前一般资料差异无统计学意义(P>0.05),但改良组术中出血、手术时间、术后住院时间均明显小于传统组,且并发症较少,差异均有统计学意义(均P<0.05)。比较两组术后视觉模拟评分法(VAS)、慢性静脉功能不全调查表(CIVIQ)、静脉临床严重程度评分表(VCsS)评分,改良组优于传统组,差异有统计学意义(均P<0.05);78例均获得随访,随访时间6~12个月,平均随访8.6个月,无严重并发症。结论改良大隐静脉剥脱术联合泡沫硬化剂治疗下肢静脉曲张是安全有效的,能够缩短手术及住院时间,减轻患者痛苦,减少术后并发症,改善术后生活质量,值得在临床上推广应用。

Objective To observe the eficacy of modified great saphenous vein strpping combined with foam sclerotherapy in the treatment of varicose vein of lower extremity.Methods 78 patients with varicose vein of lower extremity treated from January 2016 to January 2018 were selected as the research object and randomly divided into conventional surgery group and modifed surgery group,40 of whom were treated with conventional surgery as conventional surgery group while 38 treated with modified great saphenous vein stripping combined with foam sclerotherapy as modified surgery group.Perioperative stage and treatment effects were compared between the two groups.Results All patients had sucssully completed the operation,and no severe complications occurred.The preoperative general data between the two groups had no statistical significance(P>0.05).But the modified surgery group's intra-operative hemorrhage,the operation time and postoperative hospital stay were significantly less than those in the conventional surgery group,and they had less complications.The differences were staistically significant(all P<0.05).The postoperative VAS,CIVIQ and VCSS score had been conmpared between the two groups,and sores in modified surgery group were better than those in the conventional surgery group.The differences were staistically significant(all P<0.05).All 78 patients were followed up for 6-12 months with an average follow-up time 8.6 months,without serious complications.Conclusion It is safe and efective to use modified great saphenous vein sripping combined with foam sclerotherapy in the treatment of varicose vein of lower extremity.It can shorten the surgery time and hospital stay,alleviate patient's pain,reduce the postoperative complications and improve patients’quality of life after surgery which is worthy of clinical application.
