中性粒细胞计数/淋巴细胞计数比值、血小板计数/淋巴细胞计数比值在前列腺特异性抗原灰区患者中预测前列腺癌的价值 被引量:5

摘要 目的评估中性粒细胞计数/淋巴细胞计数比值(NLR)、血小板计数/淋巴细胞计数比值(PLR)在前列腺特异性抗原(PSA)灰区(血清总PSA水平4~10μg/L)患者中预测前列腺癌的价值。方法回顾性分析2018年1月至2019年6月期间郑州大学第一附属医院收治的经前列腺穿刺活检确诊为良性前列腺增生或前列腺癌的PSA灰区患者。评估前列腺穿刺活检前相关变量包括年龄、PSA、PSA密度(PSAD)、中性粒细胞计数、血小板计数、NLR、PLR与前列腺癌之间的关系。结果所有患者中前列腺癌的检出率为25.5%(60/235),良性前列腺增生的检出率为74.5%(175/235)。前列腺癌和良性前列腺增生患者NLR、PLR和PSAD比较差异均有统计学意义(P均<0.05)。NLR预测前列腺癌的曲线下面积(AUC)为62.7%(95%CI 0.555~0.699,P<0.05),PLR预测前列腺癌的AUC为61.9%(95%CI 0.544~0.679,P<0.05),PSAD预测前列腺癌的AUC为64.9%(95%CI0.578~0.720,P<0.05)。结论对于PSA灰区患者,NLR、PLR和PSAD在预测前列腺癌中具有重要价值。 Objective To evaluate the value of neutrophil-lymphocyte ratio(NLR)and platelet-lymphocyte radio(PLR)in predicting prostate cancer in the prostate specific antigen(PSA)gray area patients[serum total PSA of 4-10μg/L].Methods A retrospective analysis was performed in the First Affiliated Hospital of Zhengzhou University from January 2018 to June 2019 that the PSA gray area patients underwent prostate biopsy and the diagnosis was benign prostatic hyperplasia or prostate cancer.The relationship between prostate cancer and relevant variables before prostate biopsy was evaluated,included age,PSA,PSA density(PSAD),neutrophil count,platelet count,lymphocyte count,NLR and PLR.Results The detection rate of prostate cancer and prostate hyperplasia in all the patients was 25.5%(60/235)and 74.5%(175/235)respectively.There were statistically significant differences in the NLR,PLR and PSAD between the prostate cancer and the benign prostatic hyperplasia(P<0.05).The area under curve(AUC)of NLR was 62.7%(95%CI:0.555-0.699,P<0.05),AUC of PLR was 61.9%(95%CI:0.544-0.679,P<0.05),AUC of PSAD was 64.9%(95%CI:0.578-0.720,P<0.05).Conclusion In the PSA gray area patients,NLR,PLR and PSAD have the important value in predicting prostate cancer.
