白色念珠菌与口腔潜在恶性病变相关研究进展

Research progress on the relationship between Candida albicans and oral potentially malignant disorders

口腔潜在恶性病变(oral potentially malignant disorders,OPMDs)是指具有较高恶变风险的口腔上皮病损或状态,主要包括口腔黏膜白斑、口腔黏膜下纤维性变、口腔扁平苔藓、红斑等,严重影响患者生存质量;而口腔黏膜的白色念珠菌感染被认为与OPMDs的发生发展密切相关。已有研究发现白色念珠菌在OPMDs患者口腔黏膜中检出率高,具有较强的黏附及破坏上皮的能力,同时能够诱导宿主免疫反应,使上皮长期处于慢性炎症状态,并可代谢产生乙醛等致癌物;以上多种因素可能在OPMDs的发生发展过程中起着关键作用,且OPMDs状态下的口腔黏膜也对白色念珠菌更易感。本文对白色念珠菌和OPMDs的相关性研究进展作一综述。

Oral potentially malignant disorders(OPMDs)refer to all epithelial lesions and conditions with an in-creased risk for malignant transformation,including oral leukoplakia,oral submucous fibrosis,oral lichen planus,eryth-roplakia,etc.Additionaly,oral infection of Candida albicans is considered to be closely related to the development of OPMDs.It was demonstrated in previous studies that the detection rate of Candida albicans was higher in the oral muco-sa with OPMDs;in addition,Candida albicans showed high virulence by adhering to and destroying the epithelium.Moreover,Candida albicans was able to induce the immune response and cause chronic inflammation in the epithelium,producing carcinogenic products such as acetaldehyde.The factors mentioned above play a key role in the occurrence and development of OPMDs.Furthermore,the oral mucosa is highly susceptible to Candida albicans.The present re-view provides an introduction to the relationship between Candida albicans and OPMDs.
