关节镜下距腓前韧带修复技术的进展

Progress in arthroscopic technique for repairing anterior talofibular ligament

踝关节外侧副韧带尤其是距腓前韧带(anterior talofibular ligament,ATFL)损伤是常见的运动损伤,多数患者保守治疗有效,约有20%~40%的患者发展为慢性踝关节外侧不稳定(chronic lateral ankle instability,CLAI)。经典的Broström-Gloud手术是韧带修复的“金标准”。近年来,随着踝关节镜技术的发展,关节镜下ATFL修复的微创技术成为发展热点,尽管对镜下修复的可靠性以及神经损伤并发症等问题仍存在争议,但越来越多的研究证实镜下修复可以获得与开放修复类似的结果,且术后恢复更快。

The ankle lateral collateral ligament injury,especially the anterior talofibular ligament(ATFL)injury,is a common sports injury.Despite of the fact that most of patients are successfully treated with conservative treatment,chronic lateral ankle instability(CLAI)occurs in approximately 20%~40%of cases.The classic Broström-Gloud procedure is the"gold standard"for ATFL repair.In recent,the arthroscopic ATFL repair technique has become a hotspot.Although the reliability of arthroscopic repair technique and its complications,such as nerve injury,remain controversial,more and more studies confirm that the similar results can be obtained with either arthroscopic repair or open repair technique,whereas the arthroscopic repair achieves faster recovery than the open procedure after operation.
