D-二聚体与C反应蛋白及白细胞介素-6联合检测在下肢深静脉血栓诊断中的应用价值

The Clinical Application Value of Combined Detection of D-dimer&C-Reactive Protein&Interleukin-6 in Diagnosing Deep Vein Thrombosis

目的分析D-二聚体(D-D)、C反应蛋白(CRP)及白细胞介素-6(IL-6)联合检测在下肢深静脉血栓诊断中的应用价值。方法选取2018年1月至2019年6月于医院就诊的46例下肢深静脉血栓患者作为试验组,选取同期的46名健康体检者作为对照组,检测并比较两组入院时的D-D、CRP及IL-6水平,统计D-D、CRP及IL-6水平分别检测及联合检测的效能。结果入院时,试验组D-D、CRP及IL-6水平均高于对照组,差异有统计学意义(P<0.05)。将各外周血指标水平的检测结果作为检验变量,将患者是否发生下肢深静脉血栓作为状态变量,绘制受试者工作特征曲线(ROC)得到D-D、CRP、IL-6诊断下肢深静脉血栓的曲线下面积(AUC)分别为0.741、0.892、0.901,均有一定的诊断价值;D-D、CRP、IL-6对应的最佳阈值分别为2.601 pg/ml、24.112 mg/L、9.412 mg/L,可得到诊断下肢深静脉血栓的最佳效能;以上3个血清指标联合检测下肢深静脉血栓得到的AUC为0.941。结论D-D、CRP及IL-6与下肢深静脉血栓的形成有关,联合检测有利于下肢深静脉血栓的诊断,对病情发展及预后判断具有重要的临床实用价值。

Objective The clinical application value of combined detection of D-dimer(D-D)&C-reactive protein(CRP)&Interleukin-6(IL-6)in diagnosing deep vein thrombosis has been analyzed.Methods 46 patients with deep vein thrombosis received in the hospital from January 2018 to June 2019 were selected as the test group,and 46 individuals taking part in health examination in the same period were selected as the control group.The levels of D-D,CRP and IL-6 in the two groups were examined and compared at admission,and the diagnositic efficacy of separately detecting D-D,CRP and IL-6&combined detecting of the three indexes were counted.Results At admission,the levels of D-D,CRP and IL-6 in the test group were all higher than those in the control group and the difference was statistically significant(P<0.05).Regarded the levels of the detected indexes in the peripheral blood as the test variable and whether deep vein thrombosis has occurred as the status variable,the receiver operating characteristic curves were plotted.The areas under the curve of D-D,CRP and IL-6 in separately detecting deep vein thrombosis were obtained and they were 0.741,0.892 and 0.901,respectively and all proved to have diagnostic value.The optimal thresholds of D-D,CRP and IL-6 were 2.601 pg/ml,24.112 mg/L and 9.412mg/L,respectively and the optimal efficacy in diagnosing deep vein thrombosis could be achieved.The area under the curve was 0.941 when the three serum indexes were combined to diagnose deep vein thrombosis.Conclusion D-D,CRP and IL-6 were associated with the formation of deep vein thrombosis and the combined detection of the three indexes was demonstrated to be beneficial to the diagnosis of deep vein thrombosis,and thus showed important in estimating the development of the disease and prognosis in clinical application.
