他汀类药物对颈动脉支架置入术治疗中重度颈动脉狭窄患者转归的影响 被引量:1

摘要 目的研究颈动脉支架置入术(CAS)前他汀的应用对中重度颈动脉狭窄患者术后转归的影响.方法148例中重度颈动脉狭窄并行CAS患者按照术前是否服用他汀类药物分为他汀组(n=76)和介入组(n=72).于治疗前后检查患者的颈动脉狭窄程度;检测患者的血清hs-CRP、IL-6及血脂水平;统计患者术后不良反应发生情况;对患者随访1年以上,记录术后再狭窄的发生情况.结果治疗后他汀组颈动脉狭窄改善效果优于介入组,血清hs-CRP和IL-6水平及血脂水平均显著低于介入组(P<0.05).两组患者术后不良反应发生率无统计学差异(P>0.05).术后1年他汀组再狭窄发生率2.63%显著低于介入组19.44%;他汀组整体狭窄无进展生存期高于介入组(P<0.05).结论术前应用他汀类药物能够辅助CAS有效治疗颈动脉狭窄并改善预后. Objective To study the effect of statins before carotid artery stenting(CAS)on postoperative outcome in patients with moderate and severe carotid stenosis.Methods 148 patients with moderate and severe CAS were divided into statins group(n=76)and interventional group(n=72)according to whether they took statins before operation.The degree of carotid stenosis was examined before and after treatment.The levels of serum hs-CRP,IL-6 and blood lipid were measured.The occurrence of postoperative adverse reactions were counted.The patients were followed up for more than one year and the occurrence of postoperative restenosis was recorded.Resu Its The improvement effect of carotid stenosis in statins group was better than that in interventional group after treatment.Serum hs-CRP and IL-6 levels and blood lipid levels were significantly lower than those in interventional group(P<0.05).There was no significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05).The incidenee of restenosis in statins group was 2.63%and significantly lower than that in interventional group 19.44%.The overall stenosis progression free survival of statins group was higher than that of interventional group(P<0.05).Cone lusi on The application of statins before operation could help CAS to effectively treat carotid stenosis and improve the prognosis.
