某院静配中心利用统一审方信息系统处理医嘱的探讨

Discussion on Using Information System to Process Medical Orders in a Hospital Static Match Center

目的探讨某院静配中心统一审方信息系统对药师审核医嘱以及临床用药的辅助情况。方法通过对2019年度某院医嘱数量、系统拦截医嘱数量、药师复核后退回医嘱数量以及回退原因进行统计分析。结果2019年度某院总计医嘱数量2142714份,经系统拦截医嘱共48554份,拦截率2.27%,药师再对以上系统拦截下的医嘱进行复核后退回医嘱951份。退回主要原因包括:①溶媒问题,占比44.34%;②浓度问题,占比35.96%;③重复用药,占比10.00%;④药物相互作用,占比3.47%;⑤禁忌证,占比2.10%;⑥其它,占比0.10%。结论某院静配中心统一审方系统有效减轻了药师审核医嘱的工作量,药师可在系统上导出并统计退回医嘱的原因有助于指导临床医师再次出现同类医嘱错误。

Objective To explore the assistance of the information system of the unified examination of prescriptions in a hospital for pharmacists to examine their orders and clinical medication.Methods Through the statistical analysis of the number of orders in the hospital in 2019,the number of orders intercepted by the system,the number of orders returned after the pharmacist's review and the reasons for the return.Results In 2019,the total number of orders in the hospital was 2142714,48554 of which were intercepted by the system,with a interception rate of 2.27%.Pharmacists rechecked the orders intercepted by the above system,and finally returned 951 orders.The main reasons for return include:①vehicle problem,accounting for 44.34%;②concentration problem,accounting for 35.96%;③repeated use,accounting for 10.00%;④drug interaction,accounting for 3.47%;⑤contraindications,accounting for 2.10%;⑥others,accounting for 0.10%.Conclusion The unified prescription review system of a hospital's intravenous admixture service center effectively reduces the workload of pharmacists in reviewing orders.Pharmacists can export and count the reasons for returning orders on the system,which is helpful to guide clinicians to make similar orders again.
