术前心理干预联合程序化手术对经内镜逆行性胰胆管造影患者满意度的影响

The effect of preoperative psychological intervention combined with programmed operation on the satisfaction of the patients undergoing endoscopic retrograde cholangiopancreatography

目的探讨术前心理干预联合程序化手术用于经内镜逆行性胰胆管造影(ERCP)的可行性与有效性,分析该联合手段对患者满意度的影响。方法选取2017年2月~2019年2月本院收治的100例胆囊结石并发胆总管结石患者,根据随机数字表法分为两组,每组各50例。全部患者均接受程序化手术方案治疗[术中ERCP+内镜下十二指肠乳头括约肌切开术(EST)+腹腔镜胆囊切除术(LC)],观察组在此基础上联合术前心理干预,比较两组术后机体功能恢复情况、心理情绪、并发症及满意度。结果观察组术后各机体功能恢复用时及住院时间均较对照组短,手术当天及术后第1天焦虑评分均低于对照组,术后并发症总发生率低于对照组,对手术治疗与护理干预满意度优于对照组,差异均有统计学意义(P<0.05)。结论术前心理干预联合程序化手术的实施,利于缓解接受ERCP治疗患者的负性情绪,增强其治疗信心,患者术后机体功能恢复快,并发症少,手术实施安全性及有效性提高,护理满意度高。

Objective To explore the feasibility and effectiveness of preoperative psychological intervention combined with programmed operation for endoscopic retrograde cholangiopancreatography(ERCP),and to analyze the effect of the combined method on patients'satisfaction.Methods A total of 100 patients with cholecystolithiasis complicated with choledocholithiasis admitted to and treated in our hospital from February 2017 to February 2019 were selected and divided into two groups according to random number table method with 50 patients in each group.All the patients were treated with programmed operation scheme consisting of intraoperative ERCP plus endoscopic sphincterotomy(EST)plus laparoscopic cholecystectomy(LC).On this basis,the observation group was also given preoperative psychological intervention,and the postoperative recovery of body function,psychological emotion,complications and satisfaction were compared between the two groups.Results The recovery time of body function and hospitalization time of the observation group after operation were shorter than those of the control group,the scores of anxiety on the day of operation and 1d after operation were lower than those of the control group,the total incidence of postoperative complications was lower than that of the control group,and the satisfaction with surgical treatment and nursing intervention was superior to that of the control group,and the differences were all statistically significant(P<0.05).Conclusion The implementation of preoperative psychological intervention combined with programmed operation is beneficial to relieving the negative emotions of the patients receiving ERCP treatment and enhancing their confidence in treatment with rapid postoperative recovery of the patients'body function and fewer complications.The safety and effectiveness of the operation are improved with a higher nursing satisfaction.
