囊袋加压椎体成形术治疗胸腰椎压缩性骨折的效果

Efficacy of capsular bag compression vertebroplasty in the treatment of thoracolumbar osteoporotic vertebral compression fracture

目的探究囊袋加压椎体成形术治疗胸腰椎压缩性骨折(OVCF)的治疗效果。方法选取2018年1月~2019年9月本院收治OVCF患者80例为研究对象,按随机数字表法分为观察组和对照组,每组各40例。对照组给予椎体后凸成形术(PKP)治疗,观察组给予囊袋加压椎体成形术治疗。比较两组患者围术期指标[收缩压(SBP)、舒张压(DBP)、心率(HR)、血氧饱和度(SpO2)]差异;比较两组患者术前及术后(术后2周、术后3个月、术后6个月)疼痛程度[视觉模拟评分法(VAS)]差异;比较两组患者术前及术后6个月时健康情况[简明健康状况调查表(SH-36)]、椎体高度(前缘高度、中线高度、后缘高度)差异;比较术后6个月时两组患者并发症发生率差异。结果观察组术中SBP、DBP、HR、SpO2指标与对照组比较,差异无统计学意义(P>0.05);术后2周、3个月、6个月时,两组患者VAS评分均较术前显著降低,观察组显著低于同一时间对照组,差异有统计学意义(P<0.05);两组患者SF-36评分均较术前显著提升,观察组显著高于同一时间对照组,差异有统计学意义(P<0.05);两组患者椎体高度均较术前显著提升,观察组显著高于同一时间对照组,差异有统计学意义(P<0.05);观察组并发症总发生率低于对照组,差异无统计学意义(P>0.05)。结论囊袋加压椎体成形术可显著降低OVCF患者术后疼痛度,提升其生活质量,有利于其术后恢复,同时对维持患者受伤椎体高度也有显著作用。

Objective To investigate the therapeutic efficacy of capsular bag compression vertebroplasty in the treatment of thoracolumbar osteoporotic vertebral compression fracture(OVCF).Methods A total of 80 OVCF patients admitted to our hospital from January 2018 to September 2019 were selected as the research objects,and were divided into the observation group(n=40)and the control group(n=40)according to the random number table method.The control group was treated with percutaneous kyphoplasty(PKP),while the observation group was treated with capsular bag compression vertebroplasty.The differences of perioperative indexes(systolic blood pressure[SBP],diastolic blood pressure[DBP],heart rate[HR]and percutaneous oxygen saturation[SpO2])between the two groups were compared.The differences of pain degrees(visual analogue score[VAS])between the two groups before and after operation(2 weeks,3 months and 6 months after operation)were compared.The differences of short-form-36 health survey(SF-36)and vertebral body heights(anterior edge height,midline height and posterior edge height)between the two groups before and 6 months after operation were compared.Meanwhile,the difference of complication rate between the two groups 6 months after operation was compared.Results There were no significant differences in SBP,DBP,HR and SpO2 between the observation group and the control group during the operation(P>0.05).2 weeks,3 months and 6 months after operation,the VAS scores of the two groups were significantly lower than those before operation,the observation group was significantly lower than that of the control group at the same time,and the differences were statistically significant(P<0.05).The scores of SF-36 in both groups were significantly higher than those before operation,the observation group was significantly higher than that in the control group at the same time,and the differences were statistically significant(P<0.05).Vertebral heights of patients in both groups were significantly higher than those before operation,the observation group was significantly higher than that in the control group at the same time,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group without statistically significant difference(P>0.05).Conclusion Capsular bag compression vertebroplasty can significantly reduce the postoperative pain of thoracolumbar OVCF patients,improve their life quality,which is conducive to their postoperative rehabilitation and maintaining the height of injured vertebral bodies.
