临床药师应用PDCA循环法在内分泌科住院医嘱审核干预中的实践

Practice of Clinical Pharmacists Using PDCA Cycle Management in the Review and Intervention of the Inpatient Medical Order in the Endocrinology Department

目的探索内分泌科临床药师开展住院医嘱审核干预的思路和方法,为促进合理用药提供参考。方法运用PDCA(Plan-Do-Check-Action)循环管理法对内分泌科住院医嘱进行审核和干预,对管理前后不合理医嘱的情况进行对比,总结和分析效果,探讨医嘱审核思路,将管理流程标准化。结果通过PDCA循环管理法干预后,内分泌科住院医嘱不合格率从3.09%下降到1.01%,未针对患者肾功能遴选药物和调整剂量的不合格医嘱从10例次下降到了2例次,干预接受率从90.00%上升到了100.00%,本次干预达到了预期的目标值。结论专科临床药师应用PDCA循环管理法在住院病区进行医嘱审核和干预,是促进合理用药的重要实践,为医院促进合理用药的管理提供了一个切实可行的途径。

OBJECTIVE To explore the ideas and methods of clinical pharmacists in the endocrinology to carry out review and intervention of inpatient medical orders,and to provide reference for promoting rational drug use.METHODS The PDCA(Plan-DoCheck-Action)cycle management was used to review and intervene inpatients medical orders in the endocrinology.The situations of unreasonable medical orders before and after management were compared,and then the effects were summarized and analyzed.The ideas of medical orders’review were explored,and the management processes were standardized.RESULTS After intervention by PDCA cycle management,the rate of unreasonable medical orders decreased from 3.09%to 1.01%.The number of unreasonable medical orders that doctors did not respond to renal function selection and adjusted doses decreased from 10 to 2 cases.Intervention acceptance rate increased from 90.0%to 100.0%.This intervention reached the expected target value.CONCLUSION It is an important practice to promote rational drug use that the specialist clinical pharmacists use the PDCA cycle management method to review and intervene medical orders in inpatient wards,which provides a practical way for hospitals to promote rational drug management.
