Research progress on the implementation of discharge preparation service in aged patients

出院准备服务的实施能够保证患者在不同居住场所间所受照护的连续性,缩短患者住院时间,降低患者再入院率,提高患者和家属的满意度。本文对出院准备服务的概念、实施原则、服务内容、实施流程以及出院准备服务在老年患者中的国内外应用现状及实施成效进行述评,旨在推动出院准备服务在老年患者中的应用实践及在我国的推广实施。

The implementation of discharge preparation services can ensure the continuity of care for patients in different places of residence,shorten the length of hospital stay,reduce the rate of patient readmission,and increase the satisfaction of patients and their families.This article reviews the concept,implementation principles,service content,implementation process,domestic and foreign application status and implementation effects of discharge preparation services in aged patients,aiming to promote the application and practice of discharge preparation services in aged patients as well as their implementation in China.
